PETITION FOR CHANGE OF FIRST NAME

Petitioner, OVUVUEVUEVUE “OBI” I. CAPISTRANO, by the

undersigned counsel, respectfully states that:

1. Petitioner is of legal age, Filipino, single and a resident of Unit 1071

Tower 2 Sun Residences, Mayon cor. España, Sta. Teresita Quezon
City;

2. Petitioner was born on April 13, 1990 in Quezon City, Philippines;

3. Petitioner’s birth was recorded under registry number GV-8374018

(Certificate of Live Birth is attached here as Annex “A”);

4. Petitioner’s first name is extremely difficult to write or pronounce;

5. Petitioner has habitually and continuously used the name OBI I.

CAPISTRANO and he is publicly known in the community with that
first name as evidenced by his Employee Identification Card bearing
OBI I. CAPISTRANO as his name (attached here as Annex “B”) and
Tuition Fee Receipts issued in the same name (attached here as Annex
“C”);

6. Petitioner is under the impression that his first name is ridiculous;

 7. This petition is not in any way filed to mislead the public or for any
other fraudulent purposes and does not in any way prejudice public
interest.

PRAYER

WHEREFORE, Petitioner respectfully prays that, after due notice and

publication and hearing, this Honorable Court order the change of name of the
Petitioner from OVUVUEVUEVUE I. CAPISTRANO to OBI I.
CAPISTRANO.

Other reliefs, just and equitable under the given circumstances, are likewise
prayed for.
