AFFIDAVIT OF LOSS

I, GLEN M. ENTERINA, of legal age, single and a resident of

Bermudez Plains Subd., Apokon, Tagum City, Davao del Norte,
Philippines, after having been duly sworn to in accordance with law,
do hereby depose and say:

1. That I am the same and identical person who caused the

execution of this instrument;

2. That I am a taxpayer registered with the Bureau of Internal

Revenue under Taxpayer Identification Number 408-909-340
and as such I was issued a TIN Card;

3. That unfortunately, my BIR TIN Card bearing the said

numberwas lost and/or misplaced in my possession;

4. That despite diligent efforts exerted to locate it, the same

could no longer be found and is now considered to have been
lost;

5. That I executed this affidavit to apprise the proper authorities

concerned on the veracity of the foregoing statements and for
whatever legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set forth my hand

this 9th day of January, 2018 at Tagum City, Davao del Norte,
