Description of Activity Scaffolding Erection, Modification and Dismantling – Tank External.


Note: The tank is positively isolated, opened and cleaned prior to this risk assessment.

 Personnel carrying out scaffold erection/dismantling
shall wear full body safety harness with double
lanyard and shall be latched while working at
height.
 The safety harness shall be tied with a dedicated life
line and same shall be hooked / tied to a permanent
structure.
 Number of personnel and other materials on the
scaffolding shall be restricted as per scaffolding
load bearing capacity.
 A RED tag shall be provided while erecting /
dismantling the scaffold.
 After completion of erection of the scaffold, the
same shall be inspected and tagged. Ensure GREEN
tag on the scaffold before using it.
 Scaffold shall be inspected as per KNPC approved
checklist and certified by competent personnel in
every 7 days.
 Alteration/Modification of the scaffold shall be
carried out by competent personnel and shall be
recertified after completion.
 While utilizing portable ladders three-point contact
shall be maintained at all times.
 Personnel shall not be on any scaffold or other

temporary elevated work area during heavy rains,
storms or high winds, sustained winds more than 65
kmph (40 mph) - unless the scaffold or working
level is indoors or otherwise unaffected by the
weather conditions.
 Scaffolders and Scaffolding supervisors shall audit
on each shift of scaffolding use in order to ensure
the integrity of scaffold and ensure on gaps between
planks.
 All openings on the scaffolding platforms shall be
protected with hard barricading (guard-rails and toe-
board) or by providing safety nets.
 Provide sufficient cold water and rest work cycle for
the workers.
 Wear H2S monitor with valid calibration.
 Vehicle shall be allowed for loading and unloading
the materials and Separate Hot Work permit shall be
issued for the vehicle entry.
 Other precautions outlined in the work permit shall
be followed.

 Workers shall be mindful about the presence of the
materials lying on the ground to avoid tripping.
 Materials shall be laid in a manner that does not
block descend from scaffold.
 Perform regular housekeeping at the end of the job
ensuring no left over materials.
 For any other activity (not mentioned above), separate risk assessment will be done based on
the nature of the activity.
