The Workers Compensation Act requires that the employer must post a copy of this report in a conspicuous place at or near the workplace inspected
for at least seven days, or until compliance has been achieved, whichever is the longer period. A copy of this report must also be given to the joint
committee or worker health and safety representative, as applicable.

If this report has been issued following an incident that is under investigation by WorkSafeBC, further enforcement action may be
taken as a result of the findings of the investigation and any orders in this report.
This was a follow-up inspection to authenticate appropriate compliance with the order(s) noted.
This inspection report is for administrative purposes and results as a follow up to the employer's written Notice of Compliance
(NOC) to the orders contained in inspection report 201817946115A.
Discussions with the employer representative included the submitted NOC, and completion of the employers full incident
investigation report (EIIR), the NOC has been accepted as compliance.

ORDERS/ITEMS
An employer who fails to comply with Part 3 of the Workers Compensation Act, the Occupational Health & Safety
Regulation, or WorkSafeBC orders may be subject to monetary or other sanctions as prescribed by the Workers
Compensation Act.
An employer must immediately notify the board of the occurrence of any accident that involved the major release of a hazardous
substance.
This is in contravention of the Occupational Health and Safety Regulation Section 5.38(2).
A compressed gas cylinder must be secured to prevent falling or rolling during storage, transportation and use, and where
practicable, must be kept in the upright position.
This is in contravention of the Occupational Health and Safety Regulation Section 5.99.
An employer must ensure that an assessment is conducted of the risks posed by hazardous substances from accidental release,
fire or other such emergency.
This is in contravention of the Occupational Health and Safety Regulation Section 5.20.
The container of a hazardous substance must be designed, constructed and maintained in good condition to securely contain the
substance.
This is in contravention of the Occupational Health and Safety Regulation Section 5.98(1).
An inventory must be maintained which identifies all hazardous substances at the workplace in quantities that may endanger
workers in an emergency including hazardous products covered by WHMIS, explosives, pesticides, radioactive materials,
hazardous wastes, and consumer products.
As evidenced by;
(a) The designated storage area, a 20 foot, green metal shipping container, Serial # SP798861 was designated for the storage
of a 68 KG cylinder of ammonia. This container had not been designed, or constructed to provide for the safe containment of the
contents.
(b) There were no no signs, placards or similar means observed posted on the 20 foot, green metal shipping container, Serial #
SP798861 that would identify a hazardous substance (ammonia) was stored inside.
(d) A 20 foot, green metal shipping container, Serial # SP798861 was the designated storage area for the 68 KG ammonia
cylinder, the storage container did not have any ventilation or lighting installed.
This is in contravention of the Occupational Health and Safety Regulation Section 5.26.
