this office shall be requiring all PUBLIC and PRIVATE sihools to submit

BAslc EDUCATION INFORMATION sYsTEM (EBEls) DATA GATHERTNG FoRMs

and PSP).

z. The following procedures shall be undertaken for the puBLlc

schools:
A' Fill out the templates as directed during the orientation at paraiso Verde
Resort with correct
data.
B. Data encoded in the templates shall be varidated
by the concerned sDo officials. The
Validation Form (provided to Lls/EBEts system Administrators) as
shown below shall be

C. online encoding can be done only if the EBEIS is already open and templates are available.
Constant monitoring as to system readiness is advised.
D. Presuming that EBEIS encoding is done and assured that data in the templates are the same
with that of the encoded online, the templates shall now be printed for signatures.
E. The SDS shall sign only if the duly completed Validation Forrn is attached.
F. After the notarization of the document, a copy shall be submitted to the planning & Research
Section.
