NARRATIVE REPORT

Name: Alfred B. Porras


Age: 16 years old
Diagnosis: Visually Impaired (Totally Blind)

Work Behavior

Alfred comes to school with great disposition. He is jolly and friendly to his
classmates, peers and people around him. He expresses his needs and wants .
He travels from home to school independently. He had developed good mobility
around the campus. He follows instructions with less prompts given.

Motor Skill

Alfred had developed good gross motor and fine motor skills. He is an active
athlete in Paragames in Atheletics and Bocce. He manipulates objects with his
hands independently and develops his large muscles. He had improved his
balance and coordination and has good sense of mobility.

Academic Skill
Alfred can read and write numbers using Braille and Nemeth Method. He
can comprehend short stories and answers wh- questions. He is good in
memorization and can perform two basic operations in Math. He can read both
contracted words and uncontracted words that were transcribed in Braille.

Name: Linken Deviza


Age: 11 years old
Diagnosis: Intellectually Disabled
Placement: Full Time SPED

Work Behavior

Linken has been our pupil here in Dingle Central Elementary School Sped
Class for three years. He is a shy type kind of pupil. He comes to school with
great disposition and cooperative with his classsmates and peers. He follows
simple intruction but sometimes minimal propmts were given. He goes to school
early and worked on his assigned task with less supervision.He was less
impulsive and controlled himself if he gets frustrated.

Motor Skill

Linken had developed good gross and fine motor skills. He has been our
active athlete in Paragames competition in the field of athletics. He had
developed good balance and coordination. He manipulates things
independently.

Math Skill

Linken can perform two basic operations. He adds and subtract numbers
without regrouping in two-digit numbers. He performs addition and subtraction
through counting and can identify numbers from 1-50 in figures only with less
propmts given. He developed good concept on money and measurement.
Language Skill

Linken can identify some letters of the alphabet. He had difficulty reading CVC
pattern both in English and Filipino. He can follow 2- steps instructions but had
difficulty comprehending well. Less prompts were given during Functional Reading
activity.
