Center on August 20, 2018.
8. Attended the Pre-bid meeting with the Contractor, Technical Working
Group and BAC regarding the Construction of Acute Stroke Building
Phase I on August 23, 2018.
9. Submitted the CVMC Master Site Development Plan on August 24,
2018.
10. Attended Meeting with Chief Admistrative Officer and Engineering
Department Re: Final Design of CR Doors of the Project Five-Storey
Payward Building Phase 2 on August 28, 2018.

CONSTRUCTION MANAGEMENT/QUALITY ASSURANCE:

1. Attended Infra-meeting with Medical Center Chief-OIC, Contractor R.R.
Encabo and CVMC Engineering Department on August 09, 2018
regarding various issues and concerns of the Project: Cancer Center
Building and Five Storey Payward Building Phase 2.
2. Prepared and submitted letter dated August 29, 2018 to the Chief
Administrative Officer for the Project Five Storey Payward Building
Phase 2 regarding the following issues:
 Evaluation and recommendation for 75 days-time extension of the
said project.

3. Supervision of Five Storey Payward Building Phase II – 4 th & 5th Floor,
Four Storey OPD/MAB Building Phase I&II, Two (2) Storey Cancer
Building and Two (2) Storey Psychiatry OPD Building.

COST ANALYSIS AND ESTIMATION:

1. Prepared and Submitted the Detailed Cost Estimate and/ Bill of
Quantities of the Proposed Project: Construction of Acute Stroke
Building Phase 1.

PROPER CONSTRUCTION DOCUMENTATION

1. Along with the CVMC Engineers, issued various site instructions to
contractors on various issues and concerns in the sites.

AS-BUILT DOCUMENTS
1. Continuous update of Site Development Plan of CVMC including Water
System, Plumbing, and Drainage System.
