AFFIDAVIT

been duly sworn to in accordance with law, do hereby depose and say that:

1. I am the true and registered owner of a parcel of land located at

___________________, and covered by Transfer Certificate Title No.
_______________________

2. I acquired the property through “___________________rtgage,” and said

acquisition was entered as entry no. __________________ dated ______________.
Said entry caused the cancellation of previous title, TCT No. ______________- in lieu of
Transfer Certificate of Title No. ________________ which is now under my name
_________________________;

3. However, upon the issuance of the new Transfer Certificate of Title No.
_______________________, I noticed that there were two annotations in the new title,
which should not have been annotated therein, particularly Entry Nos. _____________-,
pertaining to the mortgage of the property in favor of _______________________.;

4. The said entries are deemed cancelled by the fact of my redemption of

the said property, as entered in the cancelled Certificate of Title No. _______________,
particularly Entry No. _______________, Date of instrument: 20 November 2013; Date
of Inscription: 30 January 2014 at 8:50 A.M;

5. Hence, the said mortgages to ______________t, as stated in the said

entries _________________ should not be included in the new Transfer Certificate of
Title No. ______________;

6. In order to reflect the true and absolute ownership of the property by the
new registered owner, I _________________________, respectfully request the
Registry of Deeds of Meycauayan, Bulacan, to remove the Memorandum of
_____________________ and ____________________ in the Transfer Certificate of
Title No. _________________________;

7. I am executing this Affidavit to attest to the truth of the foregoing, and to

be able to clear any discrepancy and/or confusion, which may have been caused by the
inadvertent annotation of Entries ______________________ in the said Transfer
Certificate of Title No. ________________________.
