I K SRIKANTH (APGENCO ID:1075576), AEE/Elec. O/o. The Project Divisional
Engineer-Generation Division, MHE(J) Scheme, Onukudelli, submit that I went my
native place(Rajahmundry) on Casual Leave for five days from 06/08/18. Unfortunately I
got severe stomach pain on 07/08/18. Immediately I was shifted to Swetha Hospital,
Rajahmundry and admitted as inpatient. There it was diagnosed as acute severe
pancreatitis. As there was no improvement in my health condition even after three days
medical treatment, in the evening on 09/08/18 I was shifted to Apollo Hospital,
Kakinada. There I was admitted as inpatient and kept in ICU till 15/08/18. On 16/08/18
morning I was shifted to General Ward and medical treatment had been continuing till to
day.

In this connection, the doctor under whom my medical treatment had been going
on suggested me to take proper medical care and rest for at least next 3 weeks. Hence
it is requested to treat my absence from headquarters as medical leave for 30 days i.e.
from 06/08/18. The necessary supporting documents are here with enclosed for taking
necessary action at your end.
