RELEASE AND QUITCLAIM

I, SHERYL RAMIREZ, Filipino, of legal age and a resident of

__________________ after having duly sworn to in accordance with law, depose and
state:

1. That I hereby acknowledge to have received the amount of

CONSULTANCY FIRM INC. as full and complete refund of the
enrollment fee I have previously advanced as stipulated in our
contract1;

2. That I hereby declare that I have no further claims whatsoever

against DOLPHIN CARE MANAGEMENT CONSULTANCY FIRM
INC.and/or any of its officers and staffs and that I hereby release and
forever discharge the same from any and all claims, demands, cause
of action of whatever nature arising out of the above-said project;

3. I further agree that this RELEASE AND QUITCLAIM may be pleaded

in bar to any suit or proceeding to which either I, or my heirs and
assigns, may have against DOLPHIN CARE MANAGEMENT
CONSULTANCY FIRM INC. and/or its any of its officers and staffs;

4. As such, I finally make manifest that I have no further claim(s) or

cause of action against DOLPHIN CARE MANAGEMENT
CONSULTANCY FIRM INC. and/or its any of its officers and staffs;
5. and forever release the latter from any and all liability.
