“A CHORUS LINE” COSTUMES RENTAL AGREEMENT

Agreement made this 21st___day of _July_____ 2014, between JMR COSTUME DESIGNS (dba Jose M.
Rivera) and _____________ _____________ _____________ pertaining to the rental of costumes for A
CHORUS LINE.

Jose M. Rivera, agrees to provide, to the above mentioned party, a complete a set of
costumes (please see enclosed inventory) for their upcoming production of A CHORUS LINE, to
be performed ___________________________________________

1. The above mentioned party agrees to pay the rental fee of ___$1500.00_________________.
There will be no rental charge for said tech time and travel time as scheduled.

Payments will be made as followed:

$375.00_____refundable deposit upon signing of this agreement. The deposit is refundable until Sept
1st, 2014 $375.00___ on Sept 1st, 2014.
$750 in January 2015 upon receiving and inspection of all costumes.

Deposit must be received within 24 hours from receiving this contract.

The above mentioned party must provide a certificate of insurance made out to JMR Costume
Designs in the amount of $50,000.00.
The insurance should cover the costumes while in transit and at the performance venue.

2. The above mentioned renting party is responsible for shipping in both directions and must
provide either a Fed Ex or UPS account number upon signing of this contract.

3. Costumes are rented “As Is”. JMR Costume Designs is not responsible for any fitting issues or
problems that may arise with said production. Costumes may not be cut or dyed and must be
 altered in such a manner that alterations may be easily removed to return garments to original
size.

4. All costumes are to be returned cleaned. Instructions are provided for the proper maintenance
of all costumes

5. Shoes, tights and undergarments are not provided and are the sole responsibility of the party
who is renting the costumes.

6. All programs must state the following:

Original Broadway Costumes Provided by JMR Costume Designs, Los Angeles, Ca

7. Costumes are needed by the above mentioned party by _______________2014 and will be
returned no later than 14 days after the final performance.

8. Costumes will be shipped from Los Angeles, California

9. If the above mentioned party agrees to these terms, please return a signed copy of this contract
