Contractor Work Permit

1 Objective
To establish a safe system of work to ensure that all hazards of risk-potential works
performed by contractors are identified, communicated and controlled

2 Scope
This procedure is applicable to all facilities of Tenaris SPIJ and Batam

A work permit must be issued for those works to be done by contractors involving:
Confined Space.
Hot Work to be done in places other than in the Maintenance Workshop area.
Scaffolding.
Excavation
Work at height.
LockOut-TagOut.
Or any other risk activity

4 Definitions and Abbreviations

Hot work: activity involving a naked flame or where high temperatures are
produced, such as welding and metal cutting. It is required a Hot Work Permit.
Contractor Supervisor is responsible for monitoring the hot work site for any unsafe
condition or for signs of small or smoldering fires. They shall have immediately
available a suitable fire extinguisher for the surrounding area and shall be fully
trained in its use. They shall remain at the hot work site for an agreed period
(typically 30 minutes but can be longer in hazardous area) after completion of work
to ensure no smoldering fires are left and to monitor for any flare-ups

Confined Space : Space has limited or restricted means of entry or exit, is large
enough for an employee to enter and perform assigned work, and is not designed for
continuous occupancy by the employee. And has one or more of these characteristics.

1. Contains or has the potential to contain a hazardous atmosphere,


2. Contains a material that has the potential for engulfing the entrant,
3. Has an internal configuration that might cause an entrant to be trapped or
asphyxiated by inwardly converging walls or by a floor that slopes downward and
tapers to a smaller cross section, and/or
4. Contains any other recognized serious safety or health hazards.

These spaces may include, but are not limited to, underground vaults, tanks,
underground pits or silo.

It is required a Confined Space Permit

Work at height: work in any place, including a place at, above or below ground level,
where a person could fall a distance liable to cause personal injury.

Shall: is used to indicate that a provision is mandatory.


Should: is used to indicate that a provision is not mandatory, but recommended as
good practice.
May: is used to indicate that a provision is optional.

6 Development

6.1 Issuing the Work Permit


The contractor with ENGI or User department, are responsible for filling in the work
permit specifying:

Description of the work to be done.


Contractor authorized to do the work.
Place and Timescale in which the work will be carried out (i.e. for how long the
permit is valid).
And fill the check list in the Work Permit:
If the answer is YES the item is applicable and the measures are Done and the work
can be conducted.
If the answer is NO the item is applicable but the measures are Not Done and the
work can not be conducted.

If the answer is N/A the item is Not Applicable in this work.


ENGI or User Department for the job, must complete the Daily Work Permit with
the list of activities to do in the area with tag out required, and people to be informed

about work in the area (i.e. Crane operator), then Maintenance Supervisor and
Operation Supervisor will have to sign the form.

The contractor shall also provide the corresponding Risk Assessment and/or Job
Safety Analysis (JSA) detailing the preventive and/or protective actions that are going
to be considered whilst performing the tasks.

Once the contractor has filled in the work permit with all mentioned information and
has attached the Risk Assessment and/or Job Safety Analysis, he/she shall sign it and
then he/she shall approach the following persons to get their approvals in the
following order:
Maintenance Supervisor/Manager.
Operation Supervisor/Manager (of the area where the job is going to be done, if
applicable).
HSE Supervisor/Manager (HSE must sign as last one person to check that all is
complete, i.e.
Contractor Safe Work Permit, Daily Work Permit, Job Safety Analysis, and check if
all the measures are OK without NO answers, and check if all the approval
signatures are done).

If the work is longer than 1 day, the Daily Work Permit must be completed every day
until Work Permit is finished to evaluate every day if the condition of work and
required new Tag Out or another safety measure.
It will be signed by ENGI or User Department, Maintenance and Operation
Supervisors.

The permit must be recognized as the master instruction which, until it is cancelled,
overrides all other instructions.

The Work Permit and Daily Permit shall be available in the area where the work is
being executed for verification and follow-up purposes.

The original form will keep by the Contractor, and the other copies will be keep by
the responsible person that signed the forms.

6.2 Validity of the work permit


The work permit is valid only once it has been signed by all related area responsible;
before that, work cannot be started.
No other work activity than that specified is authorized. If it is found that the
planned work has to be changed (scope or timescale of the work), the existing permit
shall be cancelled and a new one issued.
The work permit is valid for 30 days from the day it has been approved by all
responsible person.
The work permit can be cancelled by responsible of operation, maintenance, ENGI
or HSE in case the laid-down precautions and safety measures are not being followed
(high risk

6.3 While the work is being done

The contractor is also responsible for following and maintaining the safeguards set
out in the permit.
The Operation Supervisor, Maintenance Supervisor and/or HSE Supervisor, should
carry out spot inspections to ensure that the laid-down precautions and the safety
measures are being followed by the contractor

6.4 Work with Asbestos Containing Materials (ACMs)


Maintenance and operational activities in areas/equipments that could involve
contact with ACM shall be done according PRD05039 – Asbestos Management or
local applicable procedure
