Nurse Room:

discussing the discharge planning to be given to patient (Ny.N) with diagnosis of DHF.

PA 1: “ Good morning, Iam …. As a administer ners for today. I will give the plan of planning to patient
of Mrs. Nurul in room of Roses. From observation result, patient condition of Mrs. Nurul have
improved From laboratory result show increase of thrombocyte and hematokrit and it is normal.In
addition, the patient's physical condition is good, no longer fever.”

PP: “Is it prepared for format of a discharge planning to the patient ?”

PA 2: “I have prepared of the discharge planning for patient.”

PP: “What is the focus of this discharge planning ?”

PA 2: “Later will be given counseling about prevention dengue fever, what to consider when the
patient come home later and have a leaflet ready to take home by the patient.”

PA 1: All right then , i will discuss to a doctor about discharge planning for patient.”

Rose’s Room

Doctor and administer nurse will examine the patient with diagnose of DHF.

PA 3: “Good morning Mrs. Nurul. I am .. as a nurse.”

PA 3: “ How about your condition today mrs?”

Patient: “Good morning too, I am feel better than yesterday. I'm no longer feverish.”

PA 3: “How about your sleep last night mrs?”

Patient: I slept soundly last night. Last night I slept at 9 pm and got up at 4 am

PA 3: “Your current condition has improved, today there will be a visit from the Doctor. you will be
checked for health conditions”

Doctor: “Good morning, Mom. I heard from the nurse who took care of you, that your condition have
started to improve and from laboratory result also have showed good development, How is your diet?
Is this morning his food has been spent? "

condition of patient and the next therapy for patient.

NUM: “Doctor,about the patient in the rose’s room what should we plan to go home because of the
observation and laboratory results that indicate the patient's condition is getting better.

Doctor: “This morning i already examined condition of the patient and I already know about
laboratory result of the patient and laboratory results is indicates that the patient’s condition is
getting better. But I think the patient can’t go to the home because I wil make a sure about patient’s
condition in a few days”

NUM: “ From the laboratory results patient’s condition is more getting better and the patient only
need to adequate rest in the home.”

Doctor: “But How if the patient will fever again in the home?”

PP: “ Nurse will give health education for patient and patient’s family about prevention dengue fever
and and first handling when the patient has a fever in the home.”
