• JSA, MS & RA for safe usage of cantilever platform,
• The emergency rescue in plan.
Check:
• Visual check on cantilever platform for compliance to drawings,
• Access to the platform and the hand rail with toe board,
• The workers trained on the JSA, MS and RA,
• The over all load, including workers, less than SWL,
• The safety body harness checked and worn,
• The Live line/ anchorage point with COS from PE,
• All materials kept in containers,
• Loose materials/ tools with lanyard,
• The platform floor fully sealed and secured well,
• All loose materials cleared from the platform all the time including while
moving the cantilever platform.

Before you use the Loading Platform

Have:
• Professional Engineer’s (PE) design,
• Certificate Of Supervision by PE,
• Safe Working Load (SWL) Displayed,
• Trained personal on how to use the Loading Platform,
• Chain and the door at the end of the platform,

Check:
• Visual check on condition of the loading platform,
• The load less than SWL,
• Certified lifting crew available near the loading platform
& also at the receiving area,
• Area at the bottom barricaded with banksman,
• Areas directly above the loading platform closed off,
• No materials/ equipment are stored on the loading platform,
• Both side of the platform fully barricaded with toe board,
• Personal on the loading platform need to put on safety harness and hook to
approved anchor point.

Site Requirements

Job Safety Analysis


• All persons assigned work in elevated areas or in areas where there is a possibility of falls shall attend
and participate in JSA & Pre Start development.
• Supervisors shall analyze all tasks for fall potential and implement adequate fall prevention/protection
systems prior to work commencing.
• Supervisors shall actively question employees knowledge of the system being utilised, it’s proper
methods for use and emergency procedures associated with the task.

Fall Prevention Systems


• All team members shall utilize Primary fall prevention systems as priority for performing work in
elevated unguarded areas.
• Secondary fall protection systems should only be utilized after all efforts to employ primary fall
prevention systems have been exhausted, or when being used in concert with PRIMARY systems.

Inspection of Fall Injury Prevention Systems


• Each component of the system and its means of attachment to an anchorage shall be inspected and
tagged by a designated competent person –
• After it is installed
• At monthly intervals; and
• Immediately after it has operated or should have operated in relation to a person’s free fall at the workplace
• The identification of hazards of falling from one level to another should not only include whilst
completing a task, but should include accessing and egressing the working position.
Controls

• Identify the hazard


• Don’t have tunnel vision, look at all things that could affect you as well as the task.
• Assess the risk
• Ask yourself what if.
• Control the hazard
• What could happen if I was to carry on with the task without putting in the control measures.
• Review / update
• Once the task is complete, revisit the overall task and identify any problems that may have been overlooked
during the planning of the task.
Identify the Hazard

• A person can fall if their center of gravity is affected. Factors which may cause this include:
• Sudden acceleration or deceleration.
• Moving from one surface to another.
• The surface cannot support the load.
Identify the Hazard

• Open edges not identified or protected.


• The surface is round or slippery.
• Strong wind or rain affects the area.
• Falling or moving objects may strike a person.
• Is the person capable of carrying out the task.
Assess the Risk

• How far can they fall?


• What injuries would result?
• How often are they exposed to the hazard?
• What would cause them to fall?

Fall prevention depends upon the continued efficiency and durability of fall arrest systems and devices. It
is essential they are maintained correctly with inspections and examination of all components at regular
intervals by a competent person.
There are two types of inspection of fall arrest systems
Inspection & Maintenance

Certified Formal Inspection


• This type of inspection must be conducted by a competent person every 3 months.
• All equipment requiring inspection shall be recorded on a rigging register.
Routine Visual Inspection
• This type of inspection must be conducted by the operator BEFORE and AFTER each use.
• The operator must ensure the device is in good working order before use. Areas of concern when
inspecting basic fall arrest equipment would be?
All fall arrest equipment that has been used to arrest a free fall MUST BE REMOVED from service, tagged
out and inspected by a competent person.
The device should be destroyed if unrepairable, or not be returned to service until it has been repaired by
an authorised service centre.

Total Restraint

A control on a person’s movement by means of a combination of


a belt or harness, a line and a line anchorage which will
physically prevent the person from reaching a position at which
there is a risk of a free or limited free fall.
Types of Falls

Restrained Fall
A fall or the arrest of a fall where the person suffering the fall is
partially restrained by a restraining device such as a pole strap,
or is sliding down a slope on which it is normally possible to walk
without the assistance of a hand rail or hand line.

Free Fall
A fall or the arrest of a fall where the fall distance before the fall-
arrest system begins to take any loading, and is in excess of
600mm either vertically or on a slope on which it is not possible
to walk without the assistance of a handrail or hand line.
Features of a Harness

There are Four Main Features on how a Fall Arrest Harness works:
• Directs the load to the legs
• Keeps the body upright
• Prevents neck damage
• Slightly opens the breathing way
Correctly Fitting a Fall Arrest Harness

• Squat slightly, two inches or 50mm below groin, one palm inside.

• Can I reach the rear ‘D’ ring? Rear ‘D’ ring should be placed between
the shoulder blades at the base of the neck.

• Two palms inside for waist belt, tuck away all the loose ends.
Anchors

• If possible look for an anchor that will prevent a person from falling. Where there is the potential for a
free fall, an anchor must comply with the three following factors;
• Min b/s of 15kN (approx. 1500kg)
• Attach to anchor point as high possible above the rear ‘D’ ring if possible.
• The maximum fall distance a fall arrest harness is ‘rated’ at is 2.00 metres. How far can I fall without
coming into contact with anything.
• Anchor points are to be centrally located as possible, no more than 15 degree from centre.
• Think about what you could swing into and hit.

Attachment Devices

Hooks and ‘D’ Rings


• All attachment devices (ie. Hooks etc.) must be of the Double Action type.
• A double action hook is safe, provided that it is compatible with the ‘D’ ring or anchor point.
Attachment Devices

Karabiners
• Karabiners must be of the double action type.
• Karabiners that fall into the above category would include screw gate karabiners and twistlock (or auto
lock) karabiners.
• Clipgate karabiners, such as used by rock climbers, should not be used. When using a karabiner you
must make sure,
• The gate is screwed closed
• The screw is pointing downwards
Roll Out & Crush Out Principles

• Crush out is caused through side loading of the safety hook, this can also be seen on crane hooks.
Think about where your going to hook up
• Is there a possibility for side loading

• The “D” ring must be larger in diameter than the distance between the hook pin and the inside of the
hook end to prevent roll out.
Fall Arrestors

• Where there is a potential for a free fall, the lanyard must incorporate an energy absorber.
• Fall arresters must only be used in a fall arrest situation.
• Fall Arresters must be used vertically (Not Horizontally) and must not be locked open or leant on.
• Lanyards without an energy absorber must only
be used in a restrained fall situation.

Ladders & Steps

• Ladders are to be used as a means of access to and egress from a work area and
generally are not used to perform tasks from.
Ladders & Steps

• If a ladder is the only practical means of performing the task a risk assessment shall be submitted by
the team member to EHS Dept. for approval prior to commencing work.

• The type of tasks capable of being performed safely from a ladder are limited and at times a second
system of control, (such as using fall restraint attached to a suitable anchor point), must be used if
there is any potential for overbalancing leading to a fall.

• Portable ladders should be positioned on a substantial base and wooden blocks, bricks or off cuts
must not be used to level the feet
of any ladder. Safety feet shall be fitted.

• A pitch ratio of 4 to 1 is required and positioned in


such a manner as to have clear access top and
bottom with the top of the ladder extending
1 metre above the egress landing.

• Secured properly before use.

• All ladders extending more than 1 metre above a landing shall be braced or
secured above the landing.

• All access ladders shall have an approved safety guard rail that extends the full
length of the ladder.

• Shall have a current inspection tag.
• User must maintain 3 points of contact at all times.
• A person on the ladder must remain within the vertical
supports of the ladder.
• Items shall not be carried by hand when going up or down a ladder.
• The top 3 rungs of a ladder shall not be used to stand on.
• Personnel shall always face the ladder whilst ascending and descending.

• The motto “The right tool for the right job” is only too true concerning the use
of platform steps.
• Only an approved safety step should be used to gain the required height.
• Items such as chairs, filing cabinets, tool boxes, milk crates and old disused
drums should not be used as steps, as they often create accidents rather than
prevent them.
Emergency Procedures

• Operators using fall arrest equipment must not work alone.


• An operator suspended in a fall arrest harness should be rescued as soon as possible.
• Therefore workers and supervisor must be capable of responding to worker suspended in fall arrest
harness, or must be aware of the rescue procedures on site.
• Emergency rescue procedures must be identified in the task planning eg: JSA’s.
• All personnel should be aware that suspension in an upright condition for longer than
5 - 10 minutes can be fatal.
• This is due to a condition called “Suspension Trauma”
• Caused by gravity which results in the pooling of blood in the lower legs whilst immobile in an upright
position, and the pressure of the harness straps exerting pressure on leg veins.
• Rescue plans for all risks of fall must ensure rescue can occur within 5-10 mins to prevent this
condition.

• Fire extinguisher, designated anchorage point & Blinking light in
good condition,
• SWL clearly indicated,
• All materials kept in containers,
• Loose materials/ tools with lanyard,
• The firm & even ground,
• The safety body harness checked, worn & hooked to designated
anchorage point.
Before you use Mast Climber

Have:
• Valid Certificate of supervision by the supplier including weekly check list,
• Fire extinguisher and certified live line,
• Certified operator,
• The emergency rescue in place and working,
Check:
Visual check on live line, control panel, railing & gate locks,
• Access to the platform with hand rail,
• Fixed barricade on the ground at front and back to avoid any person to
enter the Mast climber zone,
• The entry points both on the ground and on the mast climber closed off,
• The safety body harness checked and worn,
• All materials kept in containers,
• Loose materials/ tools with lanyard,
• Confirm all doors/ Access panels could open out locked and sign in place
internally,
• Visual check on the operating elevation for obstruction/ protrusion

• Put on body harness & hooked to designated
anchorage point.
• Firm & Level Ground – Premix road / compacted hard-
core / Steel plate / Concrete floor.
• Full time banksman with visible regulating pedestrian &
aware of emergency button operation.
• Fully closed barricade.
• Blinker light, Siren, Emergency button all in working
condition.
• Fire extinguisher with good condition with in the
platform.
• Before move horizontally, SL need to drop all the way
down and all man out of lift.
Pre-Start Talks
Mast Climber Awareness Card

• Trained operator.
• Handrails, Toe boards, entry gates & entry stairs in
place and good condition. SWL displayed.
• Barricades with netting and entry gate installed.
• All exit from the building side closed with GI pipe
barricade with signage.
• All doors/ FAPs need to be sealed close with “Do not
open” sign inside.
• Valid COS for MEWP available & weekly check
confirmation available.
• Live line visible check for damages & COS for the Live
line.
• Put on body harness & hooked to Live line via sliding
mechanism.
• Emergency rescue plan in place.
• No work on top or below MEWP in operation all the
time.
