SOAP note IP STUDENT SOAP NOTE
Do not have your notes co-signed. Do ask for feedback from your residents/attendings
(consider printing out your note for this purpose).
While in the middle of writing a note, you may click “pend” if you want to come back and
finish the note later. You must sign the note for others to see it easily.
You may write and pend orders, to be co-signed by your resident/attending. You may
add info to the Hospital Summary and to the Signoff area.
For Inpatient, on the hyperspace page, choose the correct floor or Department. PROV
PEDIATRICS. For the ED, on the hyperspace page, choose “Main Campus Emergency
Department” or “MV Emergency Department.”
Be sure to fill out your CSEP on a regular basis, including asking for at least weekly feedback and
review of your CSEP. It is your responsibility to ask for feedback. If you do not receive feedback at
least weekly, email Dr. Nofziger or Dr. Knowles immediately so she can facilitate you receiving it.
Here’s another way of keeping track of things (some deliberate redundancy with above info):
Inpatient
o Night float evaluations, either drop off with Joyce Burns or hold on to until the end of the rotation – some residents may give to us personally
ED
o Write 2 H&Ps and email the patient number to Dr. Gupta (agupta@chmca.org). Do this during your first or second day in the ED. He will try
to go over them with you while you are on one of your ED shifts. You will only get an email reply if there are concerns about your
documentation.
o ED evaluations and turn in ALL evaluations to Dr. Gupta’s box/folder at the end of your shift. If you are at Beeghly during your final ED shift,
you can email a photo to Dr. Gupta and myself.
Outpatient
o Observed H&P - email Dr. Nofziger and Joyce Burns (jburns2@chmca.org) by the MONDAY of your LAST CLINICAL WEEK (the same
time your PBLI and topic question is due) that this was completed.
