SMART Goals Worksheet

Directions: This worksheet is intended to help you set SMART goals. After you have discussed what your goals should genera
scorecard or your managers goals, you would want to do the following:

1. Take each goal and ask yourself all of the following questions
2. Place an X in each box for which the question is answered by the goal you have in mind
3. Place an X in each box for which you and you manager agree on the answer to the question at hand (i.e you have had a dis
4. Answer the questions for which you have an X in either or both boxes
5. Look to see how many X's you have in each category. If you have more than half the questions IN EACH SMART CATEGOR
