TECHNICAL COMPONENT

TAB REQUIREMENTS

CLASS “A” ELIGIBILITY DOCUMENTS:

Note: The Bidders shall submit their Eligibility Documents during the Receipt & Opening of Bid Envelopes.
However, the bidder may submit the Class “A” documents to the BAC Secretariat at least five (5) days prior to
the ROBE for the issuance of certification.
LEGAL DOCUMENTS: “In case of a Joint Venture, all parties shall submit the legal documents”.

A DTI business name registration

 Sole Proprietorship
OR SEC registration certificate;
 Partnership
 Corporation

OR CDA for Cooperatives.

B Valid and Current Mayor’s Permit/Municipal License and Tax Clearance

TECHNICAL DOCUMENTS: “In case of a Joint Venture, any party of the JV shall submit the
required Technical Documents as provided below”.

C Duly signed statement of all ongoing and similar completed government and private
contracts within ten (10) years from the submission of bids, including contracts awarded
but not yet started.

D Duly signed statement of Single Largest Similar Completed Contract the amount of which
is equivalent to at least fifty percent (50%) of the Approved Budget of the Contract
(ABC), with the following attachments:

1. Notices of Award or Notices To Proceed issued by the End User or Copy of contract or
Purchase Order.

2. Certificate Of Completion or End User’s Acceptance or Proof of Payment.

 E PCAB License with at least Medium A Category B (with valid ARC registration)
*Applicable only in infra projects

FINANCIAL DOCUMENTS: “In case of a Joint Venture, any party of the JV shall submit the
required Technical Documents as provided below”.

F Audited Financial Statements for the preceding calendar year which should not be earlier
than two (2) years from the date of bid submission, stamped “received” by the BIR or
its duly accredited and authorized institutions. The Audited Financial Statements should
include the Balance Sheet, Income Statement and Cash Flow.

G Duly signed computation of Net Financial Contracting Capacity (NFCC);

OR

An unconditional Bank Commitment from a Universal or Commercial Bank to extend to it

a Credit Line if awarded the contract to be bid.

CLASS “B” ELIGIBILITY DOCUMENTS

H Valid joint venture agreement (JVA), in case of a joint venture, if applicable.

TECHNICAL DOCUMENTS

I Affidavit of Bid Securing Declaration or Bid Security (Form, Amount and Validity Period)

J Statement of compliance to Technical Specifications.

K An Omnibus Sworn Statement that: (Section 25.2.a.iv)

L Duly signed statement by the bidder that an ocular inspection had been conducted at the
project site. * For Infra Projects only

SECOND ENVELOPE – FINANCIAL COMPONENT

A Duly Signed Bid Form.

B Duly Signed Bid Prices in the Bill of Quantities.

 POST QUALIFICATION ADDITIONAL REQUIREMENTS

NOTE: The bidder with the lowest or single calculated bid shall submit the following
documentary requirements within three (3) calendar days from the receipt by the bidder of
the notice.

a) Latest Income and Business Tax Return filed through EFPS

c) Certificate of PhiLGEPS Registration

4.) BAC Resolution Declaring Bidder as the Lowest Calculated Bid/Single

Calculated Bidder.

5.) Post-Qualification Report.

6.) BAC Resolution declaring Bidder as Lowest Calculated/Single

Calculated Responsive Bid.
