SWORN STATEMENT

I, APRIL ROSE SUELTO, of legal age, Filipino, married and a

resident of Mabinay, Negros Oriental, Philippines, after being duly sworn
in accordance with law, depose and state:

That I am a housewife but sometime in June 2018, I brought a truck

of sugar cane;

That I did not personally cultivated the sugar cane;

That my gross income for the taxable year 2017 does not exceed
Three Hundred Thousand Pesos (P300,000.00).

That I have no other source of income other than the income from the
said sugar cane mentioned above;.

As such, we are executing this Affidavit to attest to the truth of the

foregoing and for whatever legal purpose this may serve.
