CONSENT AFFIDAVIT

6299 0212 3040, Hindu, Indian Inhabitant, sister of Bharat Somabhai

Solanki, residing at Navinagari, Near Railway Fatak Kherda, Anastu,

Karjan, Vadodara, Gujarat – 391240, do hereby solemnly affirm and

declare as follows:
1. I say that my father Late Somabhai Solanki, died intestate in the year

2012. Hereto annexed and marked as ‘Exhibit - A’ is the copy of the

Death Certificate dated ________.

2. I say that I am the daughter of the deceased Late Somabhai Solanki

and as such one of the legal heirs of the deceased abovenamed.

3. I say that I along with my brother Bharat Somabhai Solanki and my

sister Shanta Vasant Solanki are the only Legal Heirs and save and

except us there are no next-of-kin left by Late Somabhai Solanki.

4. I say that our mother _____________ passed away in the year ________.

Hereto annexed and marked as ‘Exhibit B’ is the copy of the Death

Certificate dated _____.

5. I say that my brother Bharat Somabhai Solanki has applied vide

Application dated ___________ for obtaining membership and getting

the Share Certificate no. _____ of Flat no. 13, B Wing, The Salomi Co-

Operative Housing Society Limited, Majethia Park, Achole Road,

Nallasopara East, Palghar – 401 209 transferred in his name.

6. I say that my I have no objection to him obtaining the membership and

transferring of the shares vide Share Certificate no. ___ of Flat no. 13,
B Wing, The Salomi Co-Operative Housing Society Limited, Majethia

Park, Achole Road, Nallasopara East, Palghar – 401 209 being granted

to my brother Bharat Somabhai Solanki and I hereby waive my rights

and accord my full and free consent to the grant of the rights, title,

interest in favour of my brother Bharat Somabhai Solanki, without

justifying my share in the estate of my deceased father Late Somabhai

Lalubhai Solanki. I also hereby waive the Notice and/intimation of the

said transfer being served upon me.
