Pseudohyponatremia: A low serum sodium concentration due to volume displacement by massive hyperlipidemia or
hyperproteinemia; also used to describe the low serum sodium concentration that may occur with high blood glucose.
For increase of 100 mg/dl glucose Na conc. falsely decreases at the rate of 2.5 mmol/L.
If a person’s blood sugar is 400 mg/dl & his Na is 127 mmol/L, corrected Na will be
127 + [(400 – 100)/100 × 2.5] = 134.5 127 mmol/L.
Causes of Hyponatremia

**Experts recommend correcting no faster than 12 mmol/L/day (0.5mmol/L/hr) to avoid Osmotic Demyelination
Syndrome (Former central pontine myelinolysis) (first calculation), and only correcting it faster — and only using
hypertonic (3%) saline — if the patient is seriously symptomatic at a rate of 1-2 mmol/L/hr (24 – 48 mmol/day) and even
then, only correcting it at 1-2 mmol/L/hr.
Management Approach
Step 1: Take the history of diarrhea, vomiting, diuretics, mineralocorticoid deficiency. If present – calculate & replace Na
NB: Na replacement process discussed later
Step 2: Does the patient have volume overload? (CCF, CLD or CKD……… secondary aldosteronism)? If so – restrict salt &
water & start diuretics.
Step 3: Measure plasma & urine osmolarity with urinary Na.
If all are normal – its pseudohyponatremia - checks for hyperproteinemia, hyperlipidemia or hypertrigliceridemia
If all are low – its primary polydipsia, confirm it by water deprivation test.
If plasma & urine osmolarity high & urinary Na normal – check blood sugar & exclude pseudohyponatrimia due to high
sugar
If plasma osmolarity is low, but urine osmolarity is not minimally low (>150), do urinary Na & check for SIADH

৩৬০ × ১০০০/১৫৪ × ২৪ = ৯৭ িম.িল./ঘ া = ায় ঘ ায় ১০০ িম.িল = িমিনেট ১০০ মাইে া প = িমিনেট ২৫ প স ালাইন
৩৬০ × ১০০০/৫১৩ × ২৪ = ২৯ িম.িল./ঘ া = ায় ঘ ায় ৩০ িম.িল = িমিনেট ৩০ মাইে া প = িমিনেট ৭.৫ প স ালাইন (িমিনেট ৭.৫
একই ভােব Ringer's Lactate = 130 Na mmol/L) এবং ½ strength Normal Saline = 77 Na mmol/L এর ইনিফউশন রট ক াল েলট
করা যােব।

Indication of 3% NaCl
1. Acute symptomatic hyponatremia
2. CKD/CLD/CHF patient with hyponatremia when intake is restricted.
3. Severe hyponatremia irrespective of symptom < 105/100 (differs from author to author)
Management of Hypernatremia
Causes of hypernatremia
1. Primary aldosteronism due to any cause (hypervolumic hypernatremia)
2. Absolute (Euvolumic hypernatremia) or relative water loss (loss of both water & Na, water > Na, hypovolumic
hyponatremia)

সমাধান
আমরা জািন একিদেন সািডয়াম ১০ িমিলেমােলর বিশ কমােনা যােবনা। সুতরাং থম িদেনর জন
স িহেসেব ১০ িমিলেমাল সািডয়াম কমােনার জন ১০/৪.২৭ = ২.৩৪ িলটার ু ইড (5% DA) লাগেব।
অথাৎ ডইিল ু ইড িরেকায়ারেমে র সােথ অিতির ২.৩৪ িলটার ু ইড (5% DA) লাগেব।

NB: পিরেফরাল লাইেন ঘ ায় সেবা ১০ িমিল মাল কের পটািসয়াম দওয়া যায়, স াল লাইেন ি ণ। 10 mmol/hr in peripheral line;
Definition of hypokalemia: Potassium level of less than 3.5 mmol/L. Normal level= 3.5-5.5 mmol/L.
মেন কির জৈনক রাগীর পটািসয়াম লেভল 2.2mmol এবং ওজন 60kg

দওয়া যােব। (তাি কভােব বার চামচ; কারন বা বতা হে এই রাগীেক আইিভ পটািসয়াম িদেত হেব)
আইিভ কােরকশান হেল ও একিদেন 90mmol K দওয়া যােবনা ৮০ িমিল মাল িদেত হেব।
সুতরাং পিরেপরাল ভইেন ঘ ায় ১০ িমিলেমাল কের আট ঘ ায় এবং স াল ভইেন ঘ ায় ২০ িমিল মাল কের চার ঘ ায় এই পটািসয়াম
আর আপিন যিদ ওয়ােড কাজ কেরন (অথাৎ আপনার কােছ িসির পা না থােক) তাহেল ২িলটার নরমাল স ালাইেন ৮০ িমিলেমাল তথা চার
( াি কািল আরও বিশ সময় িনেয় দওয়া হয়। কারন চার ঘ ায় দুই িলটার স লাইন িদেত হেল িমিনেট ১২০ প কের ু ইড িদেত হেব।
১িলটার ু ইড এক িদেন দওয়া মােন হে িত িমিনেট ১০ প কের স লাইন যাওয়া)। উদাহরণ প, আপিন যিদ মেন কেরন ২৪ঘ ায়
২িলটার ু ইেড এই ৮০ িমিল মাল ািশয়াম িদেবন তাহেল ২০ প/িমিনট কের স ালাইন িদেলই চলেব।

Causes of hyperkalemia
Approach to management
1. Calculate the corrected Ca. If its normal, no needs of Ca replacement.
2. Examine the patient. Is there any stridor, convulsion or tetany? If not symptomatic give Calcium 1g 8 hourly
3. If patient is symptomatic –
Give a bolus 200mg Calcium over ten minutes (10% 10ml Ca gluconate 22ml) Then continue Ca @ 1-2 mg/kg/hr for 6 –
12 hr
Check calcium level.
NB 10% 10 ml Ca এ িত িমিলিলটাের ৯ াম ক ালিসয়াম অথাৎ এক এ ুেল মাট ৯০ াম ক ালিসয়াম থােক।
Management of hypercalcemia
With normal or elevated PTH levels
• Primary or tertiary hyperparathyroidism
• Lithium-induced hyperparathyroidism
• Familial hypocalciuric hypercalcaemia
With low PTH levels
• Malignancy (lung, breast, myeloma, renal, lymphoma, thyroid)
• Elevated 1,25(OH)2 vitamin D (vitamin D intoxication, sarcoidosis, HIV, other granulomatous disease)
• Thyrotoxicosis
• Paget’s disease with immobilisation
• Milk-alkali syndrome
• Thiazide diuretics
• Glucocorticoid deficiency

Management of Hypomagnesemia
Magnesium is an essential element for the utilization of energy in the organic world. In green plants, magnesium is the
heart of the chlorophyll molecule that captures the energy in sunlight in order to produce oxygen and carbohydrates
(i.e., photosynthesis).
Aerobic organisms then use the oxygen to release the energy stored in organic nutrients (including carbohydrates), and
this energy is stored as ATP. The release of energy from ATP requires magnesium, which is an essential co-factor for the
ATPase enzymes that hydrolyze ATP.
Therefore, magnesium is essential for providing us with energy, and for allowing us to utilize this energy to sustain life.
Magnesium also regulates the movement of calcium into smooth muscle cells, which gives it a pivotal role in the
maintenance of cardiac contractile strength and peripheral vascular tone
Causes of Hypomagnesemia
DC
D = Diuretic therapy, Frusemide
D = Drugs aminoglycosides, amphotericin and pentamidine
The aminoglycosides block magnesium re-absorption in the ascending loop of Henley, and hypomagnesemia has been
reported in 30% of patients receiving aminoglycoside therapy.
D = Diarrhea (Secretory)
D = DM due to Mg loss in urine, Insulin induced Mg entry into cell
C = Chronic alcohol intaker
Correction
Moderate Hypomagnesemia
The following protocol is recommended for a serum Mg <1 mEq/L, or for a low serum Mg that is accompanied by other
electrolyte abnormalities:
1. Add 6 g MgSO4 over 3 hours.
2. Follow with 5 g MgSO4 over the next 6 hours.
3. Continue with 5 g MgSO4 every 12 hours for the next 5 days.
Life-Threatening Hypomagnesemia
For hypomagnesemia associated with serious cardiac arrhythmias (e.g., torsade de pointes) or generalized seizures:
1. Infuse 2 g MgSO4 IV over 2–5 minutes.
2. Follow with 5 g MgSO4 over the next 6 hours.
3. Continue with 5 g MgSO4 every 12 hours (by continuous infusion) for the next 5 days.
Just the difference is in first dose timing!
NB: Mg will always be given in diluted form.
