ABSTRACT
Guillain-Barré syndrome (GBS) is a very rare immune mediated disorder which is associated with demyelination of
peripheral nervous system and progressive muscle weakness that occurs mostly in previously healthy individuals. It
usually presents with ascending paralysis and is severe enough to warrant hospital admission for its management.
The incidence of GBS is 1.1-1.8 cases in 100,000 per year and the incidences increases with age. GBS clinical
spectrum is heterogeneous and encompasses Acute Inflammatory Demyelinating Polyneuropathy (AIDP), Acute
Motor Axonal Neuropathy (AMAN), Acute Motor and Sensory Axonal Neuropathy (AMSAN) and Miller Fisher
Syndrome (MFS). The disease is typically characterized by a rapid onset of symmetrical limb weakness, which
progresses over days to 4 weeks, and occurs in patients of all ages. Most patients also have sensory disturbances
such as tingling or dull feelings. In developed countries GBS has become the most common cause of acute flaccid
paralysis. Despite improved recognition and treatment, GBS continues to be a severe disease. Efficacious treatments
include intravenous immunoglobulin and plasma exchange but supportive care during and following the
hospitalization is also very much crucial.

Signs
On clinical examination a flaccid areflexic paralysis is
found. Muscle wasting usually occurs within two weeks
of the onset of symptoms and can be severe.

Miller Fisher syndrome (MFS) presents with ataxia,
areflexia and ophthalmoplegia. 25% of patients may
develop limb weakness. Electrophysiological studies
show primarily sensory conduction failure. Anti-
ganglioside antibodies to GQ1b are found in 90% of
patients and are associated with ophthalmoplegia. There
have been limited pathological studies in MFS but
demyelination of nerve roots has been demonstrated. A
critical difference between MFS and AIDP or acute
motor axonal neuropathy is the activation of anti-GQ1b
and anti-GT1a antibodies in MFS that target oculomotor
and bulbar nerves, which are nerves thought to have
relatively high GQ1b and GT1a ganglioside
densities(figure 3).[3]

ABSTRAK
Guillain-Barré syndrome (GBS) adalah gangguan autoimun yang sangat langka yang berhubungan
dengan demyelinatsi dari sistem saraf perifer dan kelemahan otot progresif yang terjadi sebagian
besar pada individu yang sebelumnya sehat. Biasanya disertai dengan kelumpuhan ascendens dan
cukup parah untuk menjamin masuk rumah sakit untuk manajemennya. Insiden GBS adalah 1,1-1,8
kasus dalam 100.000 per tahun dan insiden meningkat seiring bertambahnya usia. Spektrum klinis
GBS bersifat heterogen dan mencakup Polineuropati Demielinasi Akut Akut (AIDP), Neuropati
Akson Motorik Akut (AMAN), dan Neuropati Aksonik Sensoris (AMSAN) dan Sindrom Miller
Fisher (MFS). Penyakit ini biasanya ditandai oleh onset cepat kelemahan anggota gerak simetris,
yang berlangsung selama beberapa hari sampai 4 minggu, dan terjadi pada pasien dari segala usia.
Kebanyakan pasien juga memiliki gangguan sensorik seperti perasaan kesemutan. Di negara maju,
GBS telah menjadi penyebab paling umum paralisis flaksid akut. Meskipun pengobatan
ditingkatkan, GBS terus menjadi penyakit yang parah. Perawatan yang efektif termasuk
imunoglobulin intravena dan pertukaran plasma tetapi perawatan suportif selama dan setelah rawat
inap juga sangat penting.

Pengantar
Guillain-Barré syndrome (GBS) adalah polineuropati demielinasi akut (gambar 1) yang pertama kali
dijelaskan pada 1859. Ciri-cirinya adalah kelemahan motorik, disfungsi sensorik dan otonom yang
sering diikuti oleh penyakit prodromal (biasanya infeksi pernafasan atau gastrointestinal).
Diperkirakan berasal dari autoimun. GBS dapat menyebabkan morbiditas signifikan yang
membutuhkan rawat inap di rumah sakit yang lama dan periode rehabilitasi yang signifikan. Sekitar
10–15% pasien memerlukan bantuan dengan cacat sisa jangka panjang. [1]
Banyak infeksi yang mendahului telah diidentifikasi , termasuk Campylobacter jejuni,
cytomegalovirus (CMV), Mycoplasma pneumonia, virus Epstein-Barr, dan virus influenza.
Imunisasi, dan proses kelahiran juga dikaitkan dengan GBS. GBS biasanya dimulai tiba-tiba dengan
onset pareteresia distal yang relatif simetris. Pasien biasanya dapat mengidentifikasi waktu pasti
onset gangguan sensorik dan motoriK. Nyeri menonjol pada 50% pasien. Studi epidemiologis di
berbagai negara telah membentuk hubungan antara infeksi Campylobacter jejuni dan perkembangan
GBS. Infeksi Campylobacter jejuni diidentifikasi sebagai penyakit tunggal paling umum
sebelumnya pada pasien GBS dan diperkirakan bahwa hampir 25 - 40% pasien GBS di seluruh
dunia memiliki infeksi C. jejuni 1 - 3 minggu sebelum penyakit. Sebelumnya, GBS dianggap
sebagai entitas klinis tunggal. Namun, penelitian terbaru menunjukkan bahwa GBS dapat
diklasifikasikan menjadi setidaknya 4 subtipe klinis dan elektrofisiologi utama seperti polineuropati
demielinasi akut akut (AIDP), neuropati akson motorik akut (AMAN), neuropati aksonal motor
sensori akut (AMSAN) dan sindrom fisher miller (MSF). . AIDP ditandai oleh demielinasi, AMAN
terbatas pada keterlibatan motorik murni dan AMSAN adalah penyakit yang lebih berat dengan
keterlibatan sensorik motorik. Variasi subtipe GBS dan hubungannya dengan C. jejuni telah
diketahui. Reaksi rantai polymerase tampaknya menjadi alat sensitif untuk mendeteksi infeksi C.
jejuni sebelumnya pada pasien GBS. [11,12,21]
EPIDEMIOLOGI
Sepuluh penelitian melaporkan kejadian pada anak-anak (0-15 tahun), dan ditemukan kejadian
tahunan antara 0,34 dan 1,34 / 100.000. Sebagian besar penelitian menyelidiki populasi di Eropa
dan Amerika Utara dan melaporkan angka kejadian tahunan yang sama, yaitu antara 0,84 dan 1,91 /
100.000. Penurunan insiden selama waktu antara tahun 1980-an dan 1990-an ditemukan. Hingga
70% kasus GBS disebabkan oleh infeksi sebelumnya. Insiden keseluruhan GBS di seluruh dunia
adalah 1,1-1,8 kasus per 100.000 per tahun, dengan tingkat yang lebih tinggi pada laki-laki daripada
perempuan. Ada insiden usia bimodal, dengan puncak terjadi pada orang dewasa muda dan orang
tua. Insiden meningkat menjadi 3,3 kasus per 100.000 per tahun setelah 50 tahun. Ada hubungan
dengan infeksi preseden pada 70% kasus yang sebagian besar berasal dari pernapasan dan
gastrointestinal. Diduga ada hubungan antara GBS dan vaksinasi, meskipun bukti untuk ini lemah.
