Dear UK VISA APPLICANT,

Welcome to the IOM Abuja Migration Health Assessment Centre.

We would like to obtain brief information about you to enable us complete your visa application
medical form. We are requesting that you complete the questionnaire as truthfully as possible. Any
information written on this form or obtained from you during your visa medical processing will be
confidential.

Please note:

 You should have available your UK address, UK telephone number and post code.
 You should have available your present and permanent address
 For students applying for student visas who do not have UK addresses, they could use their
school’s address, telephone number and post code.
 You should also have a valid email address
 You need to know the visa type you are applying for
 You should have available your sponsor details and by sponsor we mean your financial
sponsor.
 Female applicants should answer questions 1-10 while male applicants should answer
questions 1-9
