CONTRACT OF SERVICE

Known to all men by these presents:

An agreement made by and between Mister/Miss_________________________, age

___, a resident of ________________________________________________, hereinafter
referred to as the FIRST PARTY;

and,

The Colleges of Accountancy and Accounting Technology & Teacher Education of Saint
Vincent’s College Incorporated, represented by ANGEL D. CALAGUIAN, CPA, MBA &
JOY A. ADA both of legal age, residents of Dipolog City; the Department Advisers: and,
GLAEZY A. GELOCA & SHEENA FLOR T. DAAROL, 19 & 20 respectively, residents of
Dipolog City who are the Governors of the Colleges of Accountancy, Accounting Technology &
Teacher Education for the Federation Year, 2017-2018, hereinafter referred to as the SECOND
PARTY.

That both parties have agreed upon the following premises:

SECTION I. Terms of Service – FIRST PARTY

The FIRST PARTY shall offer the following services to the SECOND PARTY
wholeheartedly and without personal reservations:

1. Attend practices so ordered by the SECOND PARTY to elude unnecessary

turmoil in the routines’ synchronizations latterly performed;
2. Perform the task of actualizing the stunts, travels and sequences preferred by the
coach/mentor, following the absolute will and desire of the SECOND PARTY;
3. Create worthy productions after every rehearsal;
4. Abide rules and regulations set by the SECOND PARTY to avoid commotions
before, during and after every practice;
5. Observe timetables so allocated by the SECOND PARTY;
6. Inform the SECOND PARTY of any inevitable deviation that may occur during
the rehearsal’s calendar, absences of the FIRST PARTY shall be strictly avoided.

SECTION II. Terms of Service - SECOND PARTY

The SECOND PARTY shall offer the following as compensation to the FIRST PARTY
for the future services to be offered:

1. Provide snacks, water and other necessities the FIRST PARTY needed during the
rehearsal;
2. Inform the FIRST PARTY of when exactly is the time their rehearsal starts;
3. Cater services and other needs of the FIRST PARTY;

4. Oversee the FIRST PARTY’s welfare before, during and after every practice;
5. In cases of emergency, the SECOND PARTY must see to it that they for always
should make themselves available to render pioneering actions on any crises
situations;
6. For ambiguity and queries, the SECOND PARTY should give the FIRST PARTY
an answer on the matters of which they are concerned.

SECTION III. Provisions of the Contract

Prior to maintain coherence, the FIRST PARTY, upon signing this Contract, is bound to
do the Services above-mentioned; so as with the SECOND PARTY in its duty and responsibility
specifically stated in Section II.

By cases of which there will be a breaching of Contract, the FIRST PARTY is thereby
reprimanded to pay an amount of 5,000.00 php to the SECOND PARTY.

SECTION IV. Termination of Contract

This contract, once made into public instrument, could never be dissolved by any means.

Either the parties violating a provision within this Contract shall permit the other to
perform legal actions against the violating party.

SECTION V. LENGTH OF SERVICE TERM

Without any occasions of premature termination of this Contract, this Contract shall be in
effect until the end of the College Day’s celebration.

SECTION IV. Effectuality Clause

This Contract shall gain effectiveness once the parties have affixed their signatures in
their own will and volition.

The term of service of the FIRST PARTY shall start once this Contract gains effectivity.

Done this 1st day of August, in the Year of the Lord Two Thousand and Seventeen, at Saint
Vincent’s College Incorporated, Dipolog City, Philippines.
