EL SÍNDROME SUBACROMIAL: LOS MELLIZOS IMPIGMENTS
En las resonancias magnéticas en hombros con impingement interno suelen aparecer: anor-
malidades en el manguito rotador, (siendo habituales desgarros entre el supraespinoso y el
infraespinoso), señal labral anormal con pequeños desgarros, cambios quísticos en la zona
posterosuperior de la cabeza humeral, y la lesión de Bennet (Calcificación en la cápsula pos-
teroinferior)1,10,15,21,38.

Aunque el diagnóstico es tarea exclusiva de los médicos, debemos saber que el dolor en la
fase final del armado, en la posición de máxima abducción horizontal y rotación externa, se
asocia al impingement interno, mientras que el dolor posterior que aparece en la fase final
del lanzamiento se asocia al dolor muscular o tendinoso del manguito rotador posterior.
Una curiosidad. Kiko tiene impingement subacromial, habiendo sido dos veces operado. Por
otro lado Jorge ha sido diagnosticado con impingement interno posterosuperior. Es curioso,
para cada mellizo un mellizo impingement diferente.
No nos importa que nos vuelvan a confundir una y cien veces más, pero sí que nos va a mo-
lestar que se confundan estos impingement porque no se conoce uno…. El “subacromial” y el
“interno posteroinferior” no paran de decir lo mismo que nosotros:

Atendiendo a los dos últimos factores que influyen en la hipovascularidad (La evolución de la
especie y la gravedad), que son quizás los grandes olvidados cuando se explica este síndro-
me, se nos ocurre una solución infalible para no formar parte de la gran Epidemia mundial
del hombro:
Es tan fácil como irnos al espacio o dejar de ser humanos. Pero para los que no podemos ha-
cer ninguna de esas dos cosas, siempre nos queda la opción de aumentar el espacio subacro-
mial, trabajar excéntricamente el supraespinoso, mejorar la disquinesia escapular, reducir el
DRIG,…. ¡A cuidar esos hombros! Que vamos a vivir muchos años, y los hombros también me-
recen disfrutar de la jubilación.

Dentro de la cintura escapular existe una casita donde vive una familia de buen nombre, entre
sus miembros encontramos algunas figuras imprescindibles en la sociedad escapular: el médico,

Cuando hablamos de la “casita subacromial” nos referimos a lo que Neer en 1972 definió como
espacio subacromial33: El suelo estaría formado por la cabeza humeral, y el techo por el acromio
y el ligamento coracoacromial.
En personas con síndrome subacromial el grosor del supraespinoso es mayor y la distancia acro-
miohumeral menor, así en un hombro sano el supraespinoso representa el 54,2% del espacio su-
bacromial mientras que en personas con este síndrome representa el 61,7%, lo que va a provo-
car que se eleve la presión subacromial31.

A veces dentro de la casita subacromial podemos tener un problema de espacio de sus habi-
tantes sin que se haya reducido, debido a un proceso inflamatorio de alguno de sus habitan-
tes, pudiendo presentar edema, fibrosis, tendinitis, tendinosis, bursitis, ….
