AFFIDAVIT

( Parental Travel Permit)

I, RONA UNABIA FLORES, Filipino of legal age, single and resident of 159 D.
Jakosalem St. Cebu City, Philippines and with mobile no.+639179200282 ,after having been
duly sworn to in accordance with law do hereby depose and state:

1. That I am the biological mother and person exercising parental authority over my minor
daughter, CHRISTINE HIEW KEI HO, 13 years old and Filipino citizen by birth;

2. That the child will travel to SOUTH AFRICA via Kuala Lumpur, Malaysia for the
following reasons:

 That her biological father, CHON TEK HO, a Malaysian citizen of legal age, has
been working in Johannesburg, South Africa as an Engineer and acquired therein
permanent resident status up to present ; and

 That the affiant and the father of my child named above previously agreed to
confer the latter full visiting rights, whenever feasible and convenient, all at his
expense at least once a year , provided that said child be accompanied by former
at all times and at all occasions during her minority;

3. That the mentioned child will be leaving on _____________________ and will stay in
the said country for a period of _________________________ and will be taken cared of
by her above named biological father during the entire duration of her stay;

4. That I cannot personally travel with my child/children due to the following reasons:

 That the herein affiant due to family circumstances, at this instance will not be
able to accompany my child to travel and visit her father as we usually did
annually ; and

 That notwithstanding my physical unavailability, I hereby expressly permit,

consent and likewise allow my child to travel alone and shall not deprive their
individual legal rights to be with each other momentarily by reason of my
absence.

5. That I am giving my full consent for the minor child to travel abroad to be accompanied
by no one except herself;

6. That I personally guarantee the financial support of the child in the course of her travel
and stay outside the Philippines and further guarantee that she will not in any manner
become public ward of any authority of any country she may happen to pass en route to
her destination;

7. That this affidavit was executed for the purpose of attesting to the truth of the foregoing
facts and for whatever legal purpose it may serve; and

8. Affiant further sayeth naught.
