The Parkland formula is the most commonly used

formula for computing the fluid replacement for burns.


The formula is 4 mL of Lactated Ringer's solution (LR) / kg weight / % TBSA (total body surface
area).
Simply put, 4 mL of LR for every kg for every %.
So 4 mL X kg X %TBSA = TOTAL VOLUME.

FOR MODIFIED PARKLAND FORMULA, SIMPLY ADD 2, 000 mL to the total volume.

1. Use the Rule of 9s in approximating the TBSA, where:


head and neck = 9%
upper extremity = 9% (2 UE= 18%)
anterior trunk = 18%
posterior trunk = 18%
lower extremity = 18% (2 L# = 36%)
genital = 1%
TOTAL OF 100%

2. If weight is in lbs., divide by 2.2.


Example, 220 lbs / 2.2 = 100 kg

LET'S TRY:
Given: weight: 100 kg
% TBSA, 50

TO COMPUTE: 4 mL X 100 kg X 50 = 20, 000 mL

3. Parkland formula requires that half of the total volume be administered within 8 h (at the time of burns), and the
remaining half of the total volume be administered in the remaining 16 h.

Half of total volume in 8 h = 10, 000 mL/8h = 1, 250 mL/h


Half of total volume in 16 h = 10, 000 mL/16h = 625 mL/h
TOTAL VOLUME given in 24 h.

4. Check time elapsed. For example, the time of burn incident is 7:00 PM. The client was brought to the ER at 9:00
PM. 2 hours have elapsed.
Because you need to infuse half of the total volume within 8 hours at the time of burns, how many
remaining hours do you need to infuse the half of total volume? The answer is 6 hours.

TO COMPUTE USING THE EXAMPLE ABOVE:


Given: weight 100 kg
% TBSA 50%
Time of incident: 7:00 PM
Time of arrival: 9:00 PM
Time elapsed: ?

Half of total volume WITHIN 8 h = 10, 000 mL/6h = 1, 667 mL/h


Half of total volume in 16 h = 10, 000 mL/16h = 625 mL/h
TOTAL VOLUME given WITHIN 24 h.
