SOCIOGRAM and DOCUMENTATION

This chapter shows the illustration of the home visits done by the student nurses
to their chosen family, these includes the interactions such as the assessment, planning,
implementation and evaluation.

FIRST HOME VISIT (NOVEMBER 23, 2017) – THURSDAY

PROBLEMS IDENTIFIED PHYSICAL ASSESSMENT INTERVIEW


The first home visit started off from the introduction of the student nurses to the
community of Barangay Ninoy Aquino and their adopted family, Soda Family. The student
nurses first met the Soda Family after a short walk at Joa ville in Brgy. Ninoy Aquino
where they received the consent to conduct the study. The Student Nurses explained the
purpose of home visits which is to help them identify the problems which exist in their
family and help them to formulate steps in the solution of their problems in the simplest
way possible. They gathered necessary information needed through observation and
interview.

The student nurses then started the interview about the Demographic profile of
each of the family members. The student nurses also gathered the information about the
socio-economic status of the family. The group also gathered the vital signs of every
individual in the family. After the accumulation of information, they gave some simple
health teachings.

When all their planned activities for the day were accomplished, the student nurses
informed the family that they had to go and shall come back for the continuation of the
Family Case Analysis.

The student nurses came back to the community and visited the Soda Family for
the second time. In the second home visit, the student nurses strengthened their building
relationship.

First, they established rapport to effectively gather more information. They


observed the family’s home and environmental condition and started to note the problems
they have identified. They performed the physical assessment of all the members present.
They informed the Soda Family about some health habits and practices that need further
teaching and assisted family members to the health teachings and necessary
interventions to promote the health condition of the family.
THIRD HOME VISIT (DECEMBER 1, 2017) – FRIDAY

HEALTH TEACHINGS EVALUATION FINAL GOODBYES


Today, the student nurses summarized all the problems seen in the family as well
as the solutions the family have identified together with the student nurses which is part
of their evaluation. The student nurses still continued giving health teachings and the
family accepted it with understanding and was able to apply the past health teachings.
They evaluated the outcome of their interventions and they expressed their token of
appreciation for letting the student nurses conduct their family case analysis.

The Soda Family is therefore equipped with enough knowledge about problems
they have identified together with the student nurse. This day marks the last day of home
visit with the adopted family and the student nurses are confidently leaving the Soda
Family. They evaluated the outcome of their interventions and they expressed their token
of appreciation for letting the student nurses conduct their family case analysis. With that,
they reinforced their teachings and went with their final goodbyes. The family showed
satisfaction and appreciation for what the student nurses are doing.

As a summary of the home visits, the student nurses started their family case
analysis by looking for a family in the community. They established a good working
relationship, with the family which enabled the students to know more about them and
enabled them to see the problems, intervene and give health teachings for the
improvement of the family’s living and health condition.
