INCIDENT/INVESTIGATION REPORT

NARRATIVE
I Officer Goodwin made contact with Trudy A. Jacobsen who was passed out behind the wheel of her running vehicle in the 100 block of
N. 3rd street. Trudy was subsequently arrested for DUI. See report for details.
On Saturday 05-23-15 at approximately 0248 hours, I (Officer Goodwin) was issuing a parking ticked to a 1998 gold
in color Toyota Camry Illinois registration , which was parked in the 100 block of N. 3rd Street after 2am.
While approaching the above vehicle I noticed it to be running with a female, later identified as Trudy A. Jacobsen,
asleep behind the wheel. I knocked on the driver window several times without a response. I shined my flash light
through the front windshield and turned it to strobe mode and knocked several more times with negative results. I
checked the driver`s door, which was unlocked and I opened the door. I shook Trudy several times and spoke to her
trying to get her to wake up. It took several moments before she awoke.
Once Trudy awoke I asked her if she was OK. She advised she was fine and that she was just sleeping. While
speaking with Trudy I observed her eyes to bloodshot and glassy. I could also smell and odor of an alcoholic
beverage on her breath. I asked her if she drank any alcohol tonight, which she advised she had. I asked her for her
Driver`s License (DL) and insurance. Trudy looked through her purse and handed me a piece of paper which was not
her DL. I told her I needed to see her DL and once again handed me a different piece of paper, which was not her
DL. I asked her to exit her vehicle to perform the Standardized Field Sobriety Test`s (SFST), which she agreed to do
so.
Upon exiting the vehicle Trudy was unsteady on her feet. I escorted her behind her vehicle and asked her to perform
the SFST`s. The first test I administered was the Horizontal Gaze and Nystagmus (HGN). I read the instructions
verbatim from a pre-printed card and asked Trudy if she understood the instructions, which she stated she did. I
asked Trudy if she was wearing contacts which she advised she was not. Prior to starting the test I asked Trudy if she
could see my finger, which she stated she could. During the test I observed Trudy`s eyes to have equal pupil size and
equal tracking. I further observed her eyes to have lack of smooth pursuit, nystagmus at maximum deviation, and the
onset of nystagmus prior to 45 degrees (with some white showing at the outside of the eye). Trudy exhibited 6 of 6
indicators of impairment. During the test I had to inform Trudy several times only to move her eyes not her head.
Trudy also was swaying while conducting the aforementioned test.
The next test I administered was the walk and turn test. Once again the read the instructions verbatim from a
pre-printed card and demonstrated the test. I asked Trudy if she understood the instructions, which she stated she
did. I asked her if she had any physical problems that would prohibit her from performing a walking or balance test,
which Trudy stated she did not. During the test Trudy could not balance in the starting position, started the test prior
to being instructed to do so. Trudy also missed heel to toe on several steps, stepped off line, and used arms for
balance. During the test at one point I thought Trudy was going to lose her balance and I stepped forward to catch
her. Trudy took ten steps and lost balance on the turn and caught herself against my squad car. I stopped the test
early for Trudy`s safety.
Through my training and experience I determined Trudy to be impaired due to alcohol and being in physical control
of the above vehicle, wherein I informed her she was under arrested for Driving under the Influence (DUI). I placed
her in handcuffs and in my squad car and transported her to the DeKalb PD.
Officer Hernandez #11 stood by while Trudy`s vehicle was towed to Lovett`s with an administrative hold placed on
it.
Once at the PD I inventoried Trudy`s belongings and escorted her do the DUI processing area. I issued Trudy
citation #189515 for DUI. At 0318 hours I read Trudy the warning to motorist and waited the twenty-minute
observation period. During that time I did not observe Trudy to do anything to invalidate the test and at 0336 hours,
Trudy submitted a breath sample, utilizing the Intox EC/IR, which I am a certified operator. The aforementioned test
indicated Trudy`s blood alcohol concentration to be .234. Trudy was issued citation #189516 for BAC greater than
.08.

I read Trudy her Miranda Rights and questioned her about tonight`s events. Trudy admitted to be had Home Town
Bar and consuming approximately 3 glasses of wine. Trudy indicated the last time she ate anything was around 1800
hours on 05-22-15.
Trudy was processed (fingerprinted and photographed) and at 0450 hours Trudy posted her DL and $100 bond and
was given a court date of 06-19-15 at 0900 hours and released.
The above SFST`s were captured on video utilizing squad camera #336. The video footage will be dowloaded to
DVD and submitted as evidence.
