AFFIDAVIT

I, SPO3 ENRIQUE PADILLA, of legal age, Filipino and a resident of

Davao City, Philippines, after being duly sworn in accordance with law
hereby depose and state:

I am the holder of LTO Non-Professional License Card No. L-01-15-

000000 issued by Land Transportation Office (LTO) and valid until August
16, 2017.

I am currently assigned in Davao City Police Station in San Pedro

Street under Task Force “TOKHANG”, introduce by the local police in order
to suppress the drug-related crimes in the city. I am assigned in the said
workplace since 2014.

On May 5, 2016, around 8:30 in the morning, I, together with SPO2

Marvin Augustin and SPO2 Rico Yan, received an information from our
informant, Leila Dayan, that certain Serena Marchesa was conducting
illegal drug activities in her house at Royal Pangi Village, Matina, Davao
City.

Acting on the said information, I, together with the abovementioned

police officers conducted surveillance-monitoring operations on her
residence, three to four times an hour on the date mentioned above. We
arrived at the residence of Serena Marchesa which is an unnumbered
house, a green bungalow-type, cemented and decorated with ornamental
plants up front.

Before the above incident, I have already have received various

information from our different informants, including Leila Dayan, that the
said house is used for selling prohibited Drugs and that Serena Marchesa
was a known Drug Pusher.

I saw more or less 20 people who were coming in and out of

Marchesa house. We observed that these people show physical signs that
they are addicted to prohibited Methamphetamine Hydrochloride, also
known as Shabu. We also actually saw some sachets of the said prohibited
drugs, weighing scale, aluminum foils, and burners.
 The said acts I personally witnessed and possession of the
mentioned articles I personally saw are clearly in violation of Section 11,
Article II of Republic Act 9165.

I am executing this affidavit to attest to the truth of the foregoing

circumstances and to apply for Search and Seizure Warrant.
