alleging that at about 10:00 PM of November 27, 2018 at Coastal Road Brgy Hinactacan La Paz, Iloilo
City, while informant was driving a private Ford Ranger Pick-up bearing conduction sticker number C1-
L137 and traversing thereat towards Jaro District when allegedly lose control of the steering wheel
while trying to evade an overtaking and encroaching single motorcycle which was travelling on
opposite direction thus, causing the pick-up to swerved to the right and accidentally bumped of the
steel post on the roadside. As a result, informant driven vehicle incurred left side mirror, left front
door, left fender and left quarter panel, left mudguard and front bumper damaged.

MONTEMAYOR ELVIE y PADERA, 46 YEARS OLD, MARRIED AND A RESIDENT OF Modern Heights
Subdivision Jar, Iloilo City with professional driver’s license number N03—04-000498, while traversing
thereat and executing to turn left heading towards Rizal Street bumped from its rear by Chevrolet
Colorado Pick-up bearing temporary plate number and registered to Benjamin A Silayro of Aurora
Subd.Iloilo City and driven by SOLEN,KERBIL y BIBILA, 28 years old, married and a resident of 1257
Purok 2, Subic Zambales with professional driver’s license number C09-10-004971. As a result, former
motorcycle rear guard dent while latter vehicle incurred left hand side bumper and fender damaged.

Joemar A. Gabales a resident of Guimaras,Iloilo and driven by LATAYADA,JAIME y TAN, 54
years old, married and a resident of Alfonso Uy, Quezon Street, Iloilo City with professional
driver’s license number F03-86-027689, while traversing thereat accidentally bumped from
its rear by Isuzu PUJ via Megaworld Mandurriao bearing plate number FVT-792 registered to
Arnel R Jagunap and a resident of Oton,Iloilo and driven by DE JUAN , NOEL BLANCAFLOR,
52years old, married and a resident of Mejorada, Street, Mandurriao,Iloilo City with

F03-01-142820 and Toyota Fortuner wagon bearing conduction sticker number AAD-7652 and
registered to Crisanta B Tumonog of Mandurriao Iloilo City and driven by CAMAYUDO REYMUNDO y
