INFORMED CONSENT FORM + COVER LETTER

Dear Prospective Participant,

Our names are Beth Sinclaire, Dana Jannetty, Gabriela Rivera, and Teanne Nguyen. We are
students completing our Bachelor of Science degree in the Dental Hygiene program at the
University of Bridgeport. This semester, we are conducting a research project which involves
surveying Registered Dental Hygienist (RDHs).

Our research title is ‘Factors Influencing Diode Laser Knowledge and Usage Among Registered
Dental Hygienists’ and the purpose of our study is to do just that. The results of this study will
help us to determine the level of understanding of diode laser usage and practice among
registered dental hygienists, attain insight of elements which can determine the extent diode
lasers are employed by dental hygienists, and ultimately improve patient care.

You are being asked to participate in this research study because you are an integral part of the
dental hygiene field that we seek to investigate. The survey consists of 16 multiple choice
questions and we ask that you please answer them in numerical order. Furthermore, the survey
should take about 15 minutes to complete. Your participation is strictly voluntary, and you may
withdraw at any time without being penalized. All responses are completely anonymous. To help
protect your confidentiality, the surveys will not contain information that will personally identify
you. The results of this study will be used for scholarly purposes only and may be shared with
University of Bridgeport representatives.

If you have any questions about this research, you may contact our supervisor Deborah
Johnson, RDH-EPP, MS at debjohns@bridgeport.edu. The results of our study will be available
after May 1, 2019 and you may receive a copy by emailing your request to our principal
investigator, Dana Jannetty at djannett@my.bridgeport.edu.

If you are a registered dental hygienist practicing in the United States, we invite you to
participate before March 15, 2019.
