AFFIDAVIT OF EXPLANATION
I, ANAMAE C. GALVE, Registrar of Philippine Call Center Institute Inc. –
ILOILO, 27 years old, married, Filipino citizen, and a resident of Barangay Alegria,
Sibunag, Guimaras, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and state THAT:

1. Late submission of PCCI (Philippine Call Center Institute Inc.) for SO Number
Request for TWSP 2018 Batch 1 to 4.;

2. Due to the reason that the student’s late submission of required

requirements.;

3. It took two to three months to collect the data and requirements of students.;

4. I am executing this Affidavit to inform the Technical Education and Skills

Development Authority Region VI, Philippines regarding for the late
submission of PCCI SO Number Request.;

5. Finally, I am executing this affidavit to attest to the truth of the foregoing

statements and for all legal intents and purposes it may serve.
