JUDICIAL AFFIDAVIT
OF KIARI KENDRELL CEPHUS

I, KIARI KENDRELL T. CEPHUS, of legal age, Filipino, single,

employee of a private company, with postal address in Apple
Avenue, Dipolog City, Zamboanga del Norte, after having been
duly sworn to in accordance with law, fully conscious that I am
answering the following questions under oath, and that I may face
criminal liability for false testimony or perjury and said judicial
affidavit was conducted at the Law Office of Atty. Krishianne
Louise C. Labiano, located at Versace Bldg., Golden St., Dipolog
City, which taking was done in English which I clearly speak and
understand and that the other information or requirements of Sec.
3, A.M. 12-8-8-SC as explained to me by the administering officer,
hereby deposes and states as follows:

PURPOSE

This affidavit of the accused, Kiari Kendrell T. Cephus is

being offered to prove the existence of doubt on the charged of
attempted homicide against the accused and deny all allegations
against him.

QUESTIONS AND ANSWERS

Question 1: Please state your name and other personal

circumstances for the records.
Answer 1: I am Kiari Kendrell T. Cephus, 28 years of age,
single, and a resident of Apple Avenue, Dipolog
City.
Question 2: Are you the same Kiari Cephus, who is charge of
this case?
Answer 2: Yes, Ma’am.

Question 3: Where were you last March 17, 2019 at about

10 o’clock in the evening?
Answer 3: I was at my home in Apple Avenue, Dipolog City
with my girlfriend.

Question 4: What is the name of your girlfriend?

Answer 4: Belcalis Marlenis Almanzar.

Question 5: What happened that night?

Answer 5: To excite herself sexually, my girlfriend
consumed Viagra before we were about to
perform sexual intercourse. When we were
carrying out the act, I noticed that she was
whimpering throughout and when we were
about to be done, she stopped breathing.

Question 6: What did you do then?

Answer 6: I called an ambulance and before we could
reach the nearest hospital, she was found dead
before arrival.

Question 6: Do you have anything to add Mr. Cephus?

Answer 6: No Ma’am.

this 19th day of March 2019, affiant personally came and appeared
with his SSS ID No. 45-823665-8 issued by the Social Security
Systems at Dipolog City, Zamboanga del Norte, bearing his
photograph and signature, known to me as the same person who
personally signed the foregoing instrument before me and
avowed under penalty of law to the whole truth of the contents of
said instrument.

have conducted and supervised the examination of the above-
named affiant at my law office in Versace Bldg., Golden St., Dipolog
City, and further declared that:

1. I faithfully recorded or caused to be recorded the

questions I asked and the corresponding answers the
affiant/witness gave; and;

2. Neither I nor any other person then present or

assisting me coached the affiant/witness regarding
the latter’s answers.
