INTRODUCTION

PTA Log Book was specially provided for students to record all the activities performed
in order to complete the PTA task.

Students are advised to refer to the PTA Guide Book to know the detail information
regarding the logbook.

The logbook is the formal way for faculty to know and evaluate the student’s attitude,
achievement and progress on PTA. Therefore, overall usage of the book is important to
be completed by each student.

IMPORTANT NOTICE TO STUDENTS

i. The logbook must be submitted to the supervisor (or assistant supervisor) for the
purpose of verification at least ONCE FOR EVERY TWO WEEKS.

ii. The logbook must be brought along on every meeting with supervisor, assistant
supervisor or any related person for verification (signature) in the “Final Year
Project Diary” section.

iii. The logbook belongs to the faculty. It must be submitted together with PTA
REPORT for evaluation purposes by the supervisor on WEEK 14.

iv. If the submission and management process of report is not followed by the
student, their final examination result will be suspended until they fulfill all the
requirement stated by the faculty. Faculty (with supervisor approval) can
withdraw the subject if they are not satisfied with the student performance.

IMPORTANT NOTICE TO STUDENT

i. In this section, students are required to record the date, time, supervisor’s or
assistant supervisor ‘s signature and meeting place related to the PTA process
from Week 1 until the submission date of PTA REPORT. Remarks of the
discussion should be stated in the provided Project Progress Summary
layout in the next page.

ii. Students will be given Grade E automatically by the faculty if the meeting
‘TIME’ was less than 5 hours.
