TO WHOM IT MAY CONCERN:

I, the undersigned guardian of Cynthia C. Ninobla, a resident of

Santiago Sur, San Fernando, La Union, understand that the curriculum for the
degree Bachelor of Science in Accountancy calls for a six (6)-unit practicum
equivalent to a minimum of one hundred (100) hours office training for
prospective graduates.

I allow my niece to undertake office practice to start on Second Semester

until she has fulfilled the one hundred hours minimum requirement. I agree to
the time schedule by the office where my child is assigned.

I also understand that certain risks are involved during the office
training. For this reason, I waive any claim against the Administration of Saint
Louis College, City of San Fernando, La Union and the Office where my niece is
assigned from any liability arising from unforeseen event.

I execute this waiver with the understanding, however, that the

adviser/administrator of Saint Louis College and/or the Commission on Audit
– Province of La Union will execute due care and diligence expected of a good
parent of a family.

Tuesday: 8:00am-5:00pm
Thursday: 8:00am-5:00pm
