It is informed that vehicle No:AP31TU 0646 (Water Tanker),Driver Name: Shaik Rafiq
has been accidented our Company Vehicle No :AP 26 CA 6579 (ISUZU MUX) at
petrol bunk Chowk in Krishnapatnam Port at 10:00hrs on 21 st March,2019.The
accident has been occurred by the water Tanker driver during his Vehicle reverse
without looking backside.

Our Company Vehicle has been heavily damaged on driver side .In this connection
vehicle photos are also enclosed here with for your ready reference. Further it is
intimated that the Water Tanker Driver is not having his Heavy Driving License.

Therefore, you are requested to file ‘FIR’ against water Tanker Driver for further
proceedings.
