I, AURORA DE VEGA ALFORTE, 47 years old, married, and

presently residing in 305 J. P. Rizal St, Tubuan I, Silang, Cavite, attest
that during the conduct of the examination for the taking of my
Judicial Affidavit in connection with Civil Case No. TG – 13-3465, I
was fully aware of the nature and consequences of the examination,
that I have answered the questions asked of me under oath, and that I
was fully aware that I may face criminal liability in case I give any
false testimony.

I was examined by ATTY. HELBERT S. DIMAS at his office

located at Dimas Law Office, Unit B Ground Floor Tapawan Bldg., E.
Aguinaldo Highway, San Agustin 1, Dasmariñas City, Cavite. The
questions asked of me as well as the answers given thereto are as
hereunder provided.
