JOINT AFFIDAVIT OF LEGITIMATION BY SUBSEQUENT MARRIAGE

We, _______________, Filipino citizens, both of legal age, married to each other, and
residents of San Fernando, Prieto Diaz, Sorsogon, after having been duly sworn in
accordance with law, do hereby depose and say:

1. That we are the parents of ABC, who was born on 1 January 2011 in
___________, copy of the Birth Certificate is hereto attached as Annex “A” and
shall form an integral part hereof;

2. That our daughter, ___________ was born out of wedlock;

3. That when our daughter, ___________ was conceived and born, we both
possessed no legal impediment to marry each other;

4. That the birth of our daughter, _____________ was registered at the Office of the
Civil Registry of Gubat, Sorsogon on ____________;

5. That on ___________, we got married to each other and our marriage was
solemnized at the Municipal Mayor’s Office in Gubat, Sorsogon by former
Municipal Mayor __________, copy of the Certificate of Marriage is hereto
attached as Annex “B” and shall form an integral part hereof;

6. That we are executing this Affidavit to attest to the truthfulness of the foregoing
statements, for purposes of complying with the requirements in applying for
legitimation by subsequent marriage of our daughter, for the proper recording of
such legitimation in her civil registry documents and to update her school records,
to enable her to bear the surname of her father and be entitled to all the rights of
a legitimated child, and for all legal intents and purposes.
