NIM : 2013.096 ABSTRACT Content: 4 Chapters, 97 Pages, 7 Attachments The background of writing this is because there are many births through the sectio caesarea as many 125 cases in Flamboyan room local public hospital of Subang Regency. The aim of this writing is to gain an authentic experience and is able to implement the nursing care directly and comprehensively to the client through the nursing process approach that involves assessment, nursing diagnose, planning, implementation and evaluation. The method used is descriptive in the form of case study. The data collecting technique are as follows: observation, interview, documentation study, and library study. Cephalo pelvic disproportion is disproportion between the size of the fetus and velvis, namely a specific size of velvis is not big enough to accomodate the outgoing of a specific fetus through velvis until the birth pervagina happens. The sectio caesarea is a an artificial birth where the fetus is pushed out through an uterine wall under one condition that the uterus is in an original condition and the weight of the fetus is more than 500 grams. The assessment to the client shows that there is a wound of sectio caesarea post-operation under the abdomen and it’s painful, sleep disorder, activity intolerance, personal hygiene disability, and lack of knowledge about the operation. The nursing plan is arranged in line with the nursing diagnose to help client to do the ralaxation technique of deep breath, set up the position a patient feel comfortable before sleep, to recommend the client to do the personal hygiene and to give heath education about feeding mother nutrition. The nursing intervention is adapted with the aim planned. After the 5 day- treatment all five diagnosis have been well overcome. However, the writer still recommends the client to see a midwife or the nearest health centre if any complaint. After the nursing care is completed the writer has a conclusion that a systematic nursing care can overcome the client’s problem. It is hoped that the health institution must be able to provide representative infrastructure and the medical team must provide themselves with nursing care science and skill.
