CONTRACT AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This contract of services executed and entered into by and between:

AMPARO C. ZAMORA herein referred to as CONTRACTEE, with an address of

Petaluma, California, USA, represented by RUBY B. SEBASTIAN with address at Butao,
Malasiqui, Pangasinan.

JOJIE SINDAYEN herein referred to as the private CONTRACTOR, with address at Brgy.
Viado, Malasiqui, Pangasinan.

WITNESSETH

WHEREAS, the CONTRACTEE is interested in engaging the services of the

CONTRACTOR for the REPAINITNG of ROOF and POSTS TRUSSES and STEEL
MATTINGS of the CATALINO D. CEREZO NATIONAL HIGH SCHOOL GYMNASIUM
located at Malimpec, Malasiqui, Pangasinan, according to the contractee’s desired specifications;

WHEREAS, the CONTRACTOR manifests that he possesses the necessary skills and
expertise to perform the said work and has agreed to perform the services herein after contracted;

NOW THEREFORE, for and in consideration of the premises and of the mutual covenants
and stipulations hereinafter set for the, the parties have agreed and do hereby agree that;

I. OBLIGATIONS OF THE PARTIES

The CONTRACTOR shall

1. Provide the necessary scaffolds and ladders for the said work including its
mobilization.
2. Provide necessary safety precautions on the work site
3. Prepare the work site for painting which includes washing (as needed), removing
of rust, removing of loose and peeling of paint, sanding and buffing, and apply
primer.
4. Place 2 coats of ______________ of _________ type of paints.
5. Clean any drips or spills and over painting and remove all CONTRACTOR’S
materials and debris from the CONTRACTEE’S work site at the completion of the
contract.
6. Finish and complete the work within 15 working days.
 The CONTRACTEE shall

1. Provide the necessary painting materials like paints and painting consumables to the
CONTRACTOR for the completion of the said project.
2. Reserve the right to have its authorized representative make periodic inspection of the
work and make necessary retouching of any portion needed.
3. Pay the contractor the amount of FORTY EIGHT THOUSAND FIVE HUNDRED
PESOS (P 48, 500.00) in the following manner:

II. CONTRACT AMOUNT

FOR, AND lN CONSIDERATION of the services to be rendered, the CONTRACTEE

shall pay the CONTRACTOR the total amount of FORTY EIGHT THOUSAND FIVE
HUNDRED PESOS (P 48, 500.00)

III. DISPUTE RESOLUTION

Any discrepancies/differences or disagreements of any kind whatsoever which shall arise

between the parties in connection with the implementation of this contract shall be resolved
amicably by both parties.

IN WITNESS WHEREOF, the parties have hereunto signed this agreement on the ______
