Please accept this letter as a formal notice of my irrevocable resignation from my

position in your company effective immediately.

It has been my pleasure to serve this company under your strong leadership and
I will forever be grateful for all the learnings and experiences I have acquired
during the many years of my employment. It is just so unfortunate that my current
physical and health condition renders me incapacitated and unfit for work for the
entire long period of medication which I am currently undergoing. As a necessary
corollary hereof, may I request for the prompt release of the full amount of my
security bond and such other pecuniary benefits by way of company policy, if
there is any, taking into consideration my years and quality of services to the
company – which funds I extremely needed to cover my expenses for my
continuing medication.
