PROJECT PROPOSAL

I. Title of the Project: “PRESERVE WATER, PRESERVE LIFE”.

II. Venue: Tayak Hill, Rizal, Laguna

III. Target Date/ Implementation: April 26, 2019 (Friday)

IV. Target Participants: DBHM Dean and faculty members, all students taking CGGSR subjects.

V. Rationale:

In order to share learning and knowledge, skills and to support our community. The Department of
Business and Hospitality Management will conduct Community Extension Program, a livelihood
program which include tree planting at the said venue.

This project will give additional knowledge and experience to the students about the importance of
helping community and taking care of our nature.

The project will be monitored and assessed.

VI. Objectives:

1. To provide environmental program benefiting Tayak, Hill.

2. To increase students interest in their local environment.

3. To improve the quality of natural environment.

4. To equip students the experience of helping community.

5. To protect the water shed.

VII. Brief Description of the Study

The activity consists of a day event which is the planting of fruit bearing trees.
The said program is part of the five year development plan of the DBHM and subject for monitoring
for 2-5 years or onward.

PARENTAL CONSENT AND WAIVER

The Department of Business and Hospitality Management in cooperation with the CGGSR class
will be conducting a Community Immersion Program at Tayak Hill at Rizal, Laguna on April 26,
2018 at 8:00 AM onwards.

In this regard students enrolled in CGGSR will be required to visit the place for the pre-need
assessment of the said place. The implementation of the project as reflected in the program of instruction
will cover the periods from April to May of the first semester.

The department will see to it that the safety of the students will be safeguarded. However, despite
the diligence of a good father, the department and school will not be held in any inevitable especially
due to negligence of the students.

Memorandum of Agreement

This agreement is and entered into this 26th day of April, 2019 at San Pablo City, by and between;

The Rizal, Laguna, represented by Hon. Don T. Concordia of legal age with the office addressed at
Rizal, Laguna. Herein refer to as the first party.

And

The Dalubhasaan ng Lunsod ng San Pablo an institution of higher learning duly organized under the
virtue of the laws of the Republic of the Philippines, with the business address at Brgy. San Jose, San
Pablo City, represented by Professor Araceli C. Adajar, Dean Department of Business and
Hospitality Management, hereafter to as the second party.

Whereas to give service for the Barangay constituents, the first party will provide the venue, material
and security to the faculty members, facilitators and students, while the latter will conduct their
activities in coordination with the Barangay Officials.

Now therefore, for and in consideration of the foregoing premises, the parties hereby agree as follows:

First Party and Second Party

1. Shall cooperatively secure faculty members, facilitators and students in the conduct of Adaptive
Community Extension Program at the Rizal, Laguna.

2. Shall jointly plan for the activity “PRESERVE WATER, PRESERVE LIFE".

3. Shall conduct extension project with the cooperation of the Barangay constituents.

4. Shall jointly monitor and evaluate the activity.

First Party

1. Shall provide the venue for the DBHM Extension outreach program.

2. Shall provide other materials for the Extension outreach program.

3. Shall provide security for the volunteer students and lecturer during the implementation of the
implementation outreach program.
Second Party

1. Shall provide manpower for the barangay areas and other related activities and share facilities
which can help conceptualize the project.

2. Shall carry out monitoring and evaluation of the project in the service area.

3. Shall submit reports to the first party regarding the outcome of the activity.

This agreement shall cover the period of April 26, 2019 and unless sooner terminated by any parties
herein for any cause, provided that the prior notice to the intentions to terminate shall serve upon the
party.
