CONTRACT OF PROFESSIONAL SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This Contract for Professional Services is made and entered into by and between:

The CITY GOVERNMENT OF TAGBILARAN, a public corporation organized and existing pursuant to its Charter, herein represented
by the Honorable City Mayor, JOHN GEESNELL L. YAP II, of legal age, Filipino, married and a resident of Barangay Dao, Tagbilaran City,
Bohol, Philippines, pursuant to Appropriation Ordinance No. 18-05, enacted on November 16, 2018 and hereinafter referred to as the FIRST
PARTY;

- and -

FRANCIS ROLETH BALO, Filipino, of legal age and a resident of Purok 6, Cubacub, Mandaue City, Cebu, Philippines, hereinafter referred
to as the SECOND PARTY;

WITNESSETH

1. That the FIRST PARTY hires the SECOND PARTY as the Official Violin Instrumentalist for the Mutya sa Tagbilaran 2019 Grand
Coronation Night on April 26, 2019 at Bohol Wisdom School Gymnasium, Tagbilaran City;

2. That the FIRST PARTY shall provide Food, Transportation and Accommodation;

3. That the FIRST PARTY shall pay the SECOND PARTY the amount of Eight Thousand Pesos (P 8,000.00) only and to be deducted
by the 13% of Tax;

4. That the FIRST PARTY shall pay the SECOND PARTY in full after the show;

5. That the SECOND PARTY shall attend the Technical Rehearsal on April 25, 2019 from 9:00am to 12:00mn at the Bohol Wisdom
School Gymnasium;

6. That the SECOND PARTY should comply with all the rules stated herein. In case of non-compliance; the Talent will pay for damages;

7. That in case of cancellation or abandonment of the contract due to the causes which are classified as fortuitous events, force majeure or
“Act of God” or other similar incidents, the FIRST PARTY and SECOND PARTY shall agree to subsequent negotiations,
