Summary. Male 18 years old, single, who travelled to Juliaca-Puno-Peru (3824 m asl),
arriving at its destination at 02.00 hours on 09.02.2015, presenting 1 hour after intense
epigastric pain associated with cramps and food vomiting, for what he is admitted to a
hospital in Juliaca. 59 hours (13.00 pm on 11.02.2015), he is subjected to an
abdominal ultrasound (spleen and liver: slightly increased in size), treating him with
painkillers discharging him 1 day later. Hours later, reappeared abdominal pain,
now located in the left upper quadrant, being readmitted in the same hospital, where
he remained for four days. Later he is referred to Dos de Mayo National Hospital:
HNDM (02/17/2015), where he looks conscious and oriented, presenting a palpable
spleen at 2 cm below the left costal margin and positive percussion lumbar left fist.
The 20/02/2015 his abdominal CT with contrast shows: splenic infarction and massive
hepatomegaly, Hemoglobin electrophoresis ;Hb S: 37.9%, fetal Hb 4.3%. Because of
fever (37,9oC) he received antibiotics, hyoscine and other improving, reason for which
he was discharged 7 days later. The 03/21/2015: the patient receives his first dose of
meningococcal vaccine. 10 months later (01.25.2016), the patient is subjected to an
abdominal CT contrast, showing: decreased splenic volume with some normal areas
and some segmental areas that capture few contrast dye.

REPORTE DE CASO
frasco c/12 horas (1 día), mejorando, siendo dado de alta 7 días después. 21.03.2015:
paciente recibe primera dosis de vacuna antimeningococica.
Antecedentes: Tabaco y alcohol ocasional sin embriagarse, desde los 17 años. Padeció
de asma entre los 2 a 9 años, actualmente sin tratamiento. Madre, natural de Lima, ha
padecido de pancreatitis aguda litiasica y TBC pulmonar curándose tras recibir
tratamiento completo. Padre aparentemente sano. Abuelo paterno nació en Chincha,
enclave de migración africana en el Perú
20.02.2015: Rx. Tórax AP y lateral: Normal. TAC abdominal, con contraste (Hospital de
Emergencias Pediátricas de Lima): infarto esplénico extenso (masivo) y
hepatomegalia.31.12.2015: 20.02.2015. Hematíes: 5,200 000/mm3, Hb: 14 g/dl,
MCV:78 fl, Leucocitos: 8200 mm3, neutrófilos:5382/mm3, linfocitos: 1076/mm3.
Monocitos: 1078/mm3, eosinofilos: 662/mm3, plaquetas:668 000/mm3. Proteínas
totales: 7,04 g/dl, seralbuminas: 4,21 g/dl, seroglobulinas:2,83 g/dl, Bilirrubina
Total:1mg/dl, Bilirrubina indirecta:07mg/dl, Fosfatasa alcalina: 74 U/L, GOT:22 U/L,
GPT:30 U/L, GGGT:15, glucosa: 77 mg/dl, urea :18 mg/dl, Na, K, Cl y test HB S :39,7 U,
IgG (normales).
