I, SPO1 VANDA CHARISSA T. DAYAGBIL, of legal age, Filipino,

single and a resident Jakosalem St., Zapatera, Cebu City, after having
been sworn in accordance with law hereby depose and state, that:
 PRELIMINARY STATEMENT

The person examining me is Atty. Cristine Mae Villablanca with office

address at SVED Law Tower, Cebu Business Park, Cebu City,
Philippines. The examination is being held at the same address. I am
answering the questions asked of me by Atty. Villablanca fully
conscious that I do so under oath and that I may face criminal liability
for false testimony or perjury.

This judicial affidavit is written in the English language that is known

to me.

My examination in question and answer form is as follows:

legal age, Filipino, single, and a resident of Jakosalem St.,
Zapatera, Cebu City. I’m a Police and a Record Custodian.

A: I was at the Police Station.

3.Q Where was this Police Station located?

:
A: This Police Station is in Talamban where Juan Dela Cruz
was invited.

4.Q Do you know Juan Dela Cruz?

:
A: I don’t personally know him but I came to know him when
he went to Talamban Police Station on September 20, 2018.
5.Q How can you be so sure that he was Juan Dela Cruz?
:
I’m
