Journey to Germany Document Checklist

Document Needed to File Your Nurse Recognition or License in Germany

You may start gathering all the required documents and submit the documents stated below, if you
have met the 2 conditions:
signed the MGSI-Nurse Program Agreement

enrolled in A2 German language level

Documents for translation:

1. Original Passport valid for 18 months

2. Passport Information Page
3. PSA Birth Certificate (NSO not accepted anymore)
4. PSA Marriage Certificate for female only (if applicable) (NSO not applicable)
5. Bachelor of Science in Nursing (BSN) Diploma
6. BSN Transcript of Records (TOR)
7. RLE
8. Nursing Board Certificate
9. PRC Certificate of Good Standing
10. PRC Nursing License (Front & Back) in one page only
11. PRC Board Rating
12. Certificate of Employment(s) with a short description of your tasks signed by your immediate
supervisor with full contact details for verification. NOTE: If you have an old one, this is ok
for now until the local authority requests.
-Volunteer Nurse in a hospital
-COE RN Heals COE
-Private Duty Nurse COE with name of patient, diagnosis, attending doctor’s name,
dates of
employment, detailed description of your tasks, contact person and contact
number of the person who hired you as a PDN
-Company Nurse COE with the job description
- COEs of Hospital abroad
C e r t i f i c a t e should indicate the following: (1) position
(2) Department (3) start and end date of employment (4) full-time or part-time
nurse (5) description of tasks, example of tasks (administrating medication,
vital signs monitoring, bedside care: personal hygiene, etc.
13. NBI (travel abroad only) 6 months valid at time of submission of documents.
NOTE: Before deployment, a new NBI needs to be secured.

Reminder:
1. Make COLORED copies in an A4 sized white paper
2. Reduce the size if necessary to capture everything.
3. Notarize all documents. If copies are certified true copies, no need to notarize.

4. Scan all your notarized and certified copies of documents. Create your Dropbox account
www.dropbox.com using your email address. Upload all documents in your dropbox account; name
documents with your FullName Name of Doc. Share your folder to lance.raphael16@yahoo.com;
gg.degala@magsaysay.com.ph
5. Germany Local Authority requires only photocopies in A4 sized white paper but the notarized
stamp must be original.
6. Print the document checklist below and place all your documents in a brown long envelope with
plastic envelope.
7. Handcarry or send via LBC at

translator and you pay directly Php10,000 as down payment.
