Chapter 1 - Introduction
1.1 General

Government of India had declared the National waterway NW-5 in Mahanadi / Brahmani delta,
Matai River & East Coast Canal (ECC) in November 2008 for total length of about 600 km. The IWAI
has taken up action for implementing phase 1 development of 212 Km National Water way no 5
between Pankapal to Paradeep and Dhamra and therefore intend to examine the cross-drainage
structures such as bridges (except railway bridges) for adequacy of required horizontal and vertical
clearance for class-III waterway and suggest modification/reconstruction plan including detail
engineering report.
Details of the existing bridge
The bridge at jokadia is located over river Kharasrota connecting the roads between Jokadia and
Kanhupur in the district of Jajpur. The bridge is located at distance of 11.700 km from Pankapal
(considering Pankapal as 0.000 km at NH-5 A). The existing bridge of 7 spans of 41.5 m c/c with 5.5 m
carriageway width was constructed in the year 2006,. All the foundations are of well foundation
having 7 m dia well for abutments and 6.0 m dia. well for piers.
The existing bridge is of wall type substructure for pier shafts and abutments, with Roller
rocker bearing & RCC box type Girder with RCC deck superstructure. The wearing coat of the bridge
is of cement concrete wearing coat with surface reinforcement.

1.2 Scope of work


The assignment comprises of finalisation of the concept, preparation of GAD, carrying out site
investigations culminating in preparation of Detailed Engineering Report which includes Detailed
Engineering Design and Drawing for modification or reconstruction of existing Bridge over river
Kharasrota. Preparation of QAP is also a part of the scope of works.
Construction of HL bridge over river Kharasrota is totally mechanized involving group of piles
as foundation under pier and abutment locations and steel through type girder in super structure.
The quality assurance plan prepared for this project contains procedures and guide lines to
be followed by the contractor’s engineer and supervision personnel. The tasks are mentioned for
inspection, testing and reporting. The manual is a base document outlining Quality Assurance Policy,
responsibilities compliance, acceptance criteria and documentation.
The Quality Assurance Plan also outlines duties and responsibilities of the Quality Control
Personnel deployed by the Construction Agency/Supervision Consultant/Client for execution of the
project. This document will help in providing a strong and durable building of assured quality.

2.2 Quality Control Personnel


2.2.1 Senior Material Engineer.
Qualification & Experience
He shall be graduate in civil engineering from a recognized university. He shall have
professional experience of 12 years in construction of highways/roads/bridges. He shall have
experience of 5 years in similar capacity in construction/construction supervision of
highway/bridge projects. He shall have experience as Material/Geotechnical Engineer in
construction/ construction supervision at least 2 highway projects. He must be familiar with
material property of road construction material, technical specifications and procedures of
material tests and testing equipments. He shall preferably have post graduate degree in
Geo-Technical Engineering/Soil Mechanics and Foundation Engineering. He shall also
preferably have experience as Material Engineer in highway construction projects outside
the country. He shall not be more than 65 years of age.

Role & Responsibility


i. Shall remain responsible for all Quality Assurance, Quality control and Acceptance
requirements and will directly report to the project manager.
ii. Regularly supervise the activities of QC/Asst. QC Engineer.
iii. Provide all necessary assistance to the construction team.
iv. Providing inputs to project manager for preparation of work methodology.
v. Setting up of field laboratories
vi. Calibration of equipment
vii. Preparation of JMF for flexible pavement and concrete mix design
viii. Maintain an effective documentation system for QA/QC throughout the project
ix. To monitor and coordinate quality control activities on site
x. Ensuring all the test shall be carried out as per relevant IS/applicable code of practice
xi. Maintain all the test records for all the construction material used in the project.
xii. Verify the system for reporting and disposing non-conformance and corrective action
requests.
xiii. Have final say about acceptance of a material/work
xiv. Perform internal quality audits
xv. Liaising with suppliers, customers and manufacturing/development staff.

2.2.2 Material cum Quality Monitor.


Qualification & Experience
He shall be Graduate/Diploma in Civil Engineering having minimum 2/5 years of work
experience (2 year in case of graduate and 5 years in case of diploma) in the field of quality
control in road/bridge construction projects. He shall Familiar with testing of different
materials/finished products, testing apparatus, calibration, sampling, testing, recording test
observations in proper formats and determination of test results.
Role & Responsibility of Material cum Quality Monitor.
i. Shall be directly responsible for quality control.
ii. Reporting to the Sr. Material Engineer.
iii. Supervising the activities of the Lab Technician.
iv. Assisting Material Engineer in setting up the field laboratories.
v. Supervising the work of Lab Technicians.
vi. Ensuring proper calibration of all laboratory equipment.
vii. Conducting laboratory and field tests on random basis.

2.2.3 Lab Technician


Qualification & Experience
He must have Diploma in Civil Engineering/Science graduate/ITI having minimum 2/5 years
of work experience (2 years in case of Diploma and 5 years in case of Science graduate /ITI)
in the field of testing of materials.
Role & Responsibility of
i. Work under the guidance of QC/Asst. QC Engineer.
ii. Sampling of construction materials and preparation for testing as per IS code.
iii. Conducting laboratory test of construction materials.
iv. Inspection of batching plant, hot mix plant and WMM plant at the time of production.
v. Conducting field tests at site and recording test observations.

Chapter 3 - Field Laboratory Set up


3.1 General
Quality Control requires testing of materials, processes and workmanship in various stages of
construction. Adequate number of field laboratories shall be set up by the Contractor at the project
site by installing the equipments suggested in section 100 of the MORTH Specifications for Road and
Bridge Works. The number and size of laboratories shall be as per the crushers, concrete batching
plant, wet mix plant, hot mix plant and the camp office. If all are located in the same place, one
single laboratory with the facilities to test all the construction materials shall be established in the
camp site. If the plants are located in different locations, small plant laboratories shall be established
at the respective plant site for conducting corresponding tests, to cater to the requirements of
individual plants.
The General Features of the laboratory shall be as follows:-
i) At the project site, there shall be a central/main laboratory.
ii) Plant laboratories shall be established if they are located in different locations than the main
camp site.
iii) Test equipment at each plant laboratory shall be specific to the plant established at that
location.
iv) The layout of the laboratory shall be as per the test facilities required at that location. The test
facilities are divided in to three principal sections namely, soil and aggregate testing section,
concrete section, asphalt section.
v) The plan laboratory shall have a concrete section for concrete batching plant and aggregate &
asphalt section for wet mixed macadam and hot mix plants.
vi) The list of equipment and various accessories in each laboratory shall be on the type of
laboratory and the test facilities planned by the contractor at that location.
vii) The main/central laboratory shall be headed by the Quality control/Asst. Quality control
Engineer.
viii) All the tests shall be conducted as per relevant IS codes of practice.
ix) In the absence of IS for any test, reference may be drawn from any other appropriate
international code.
x) All the test results shall be properly documented.
xi) All the test formats shall be used for recording the results of various tests.
xii) The contractor shall be carried out all the tests in their own laboratories.

xiii) If the contractor do not have own laboratories, they shall arrange to conduct the tests in the
laboratories of reputed academic institutions or in accredited third party laboratories.
xiv) During construction, the contractor shall conduct tests on every materials which shall be used in
the project including raw materials, processed materials, manufactured materials, proprietory
materials, etc.

3.2 Up keep & Maintenance


i) Laboratories shall be adequately lit, ventilated and well maintained with proper water supply
and sanitary arrangement.
ii) Equipment/apparatus requiring temperature controlled environment shall be housed in air
conditioned room.
iii) Tests that using inflammable substances, shall be conducted in separate room having adequate
ventilation.
iv) All the equipment shall be checked periodically for their functioning and serving the intended
purpose.
v) Proper maintenance shall be carried out at fixed intervals through authenticated service
provider.
vi) Calibration of measuring equipment shall be carried out as per specified interval.
vii) Any inspection, measurement and testing equipment that undergoes excessive wear and tear
and requires calibration more frequently than the frequency recommended by the
manufacturer shall be replaced.
viii) . All the dial gauges, pressure gauges and similar equipment, whose actual readings vary by
more than 25% of the indicated readings shall be replaced.
ix) Equipment which may be more than 15 years also may be considered for replacement unless it
is certified by the accredited laboratories to be fit for use.

3.5 Frequency of testing.


The first step towards ensuring quality construction is quality of raw materials like Aggregates, Sand,
Earth or Water or processed manufactured materials like Cement, Admixture, Reinforcement steel,
etc in construction activities. All the sources shall require prior approval of the source or supplier for
the individual material or product.

in order to ensure that material used in construction is of high quality and meets the codal
requirements of BIS, IRC or MORTH as applicable, a series of tests may have to be carried out at
regular frequencies. The testing has to be done first at the level of the supplier/manufacture or the
contractor. This forms the first level check. The testing may be done jointly with the client or his
consultants in the site laboratory. This forms the second level check. The material may sometimes
be tested at reputed Govt. approved independent laboratories. This forms third level check.

The required test for the various materials as well as the frequency of such testing and
acceptance is listed below:-

Chapter 4 - SUPERVISION METHODOLOGY AND


DOCUMENTATION
4.1. Method of Supervision.

a. The contractor shall submit the RFI (Request For Inspection) in approved format prior to 24
hours for the item of works he intends inspection by the Consultancy team.

b. The RFI should contain the Sl.No of Book and to be maintained with serial No of pages with
triplicate copy of different colours.

c. The 1st copy of RFI is to be submitted by the agency to the Consultancy team before
concealing any item of works.

d. The concerned field officer shall attend the work for supervision/checking of the work with
RFI.

e. The concerned Field officer shall check the work and put his signature as token of approval
of the work.

f. In case not approved, the reasons for non-approval are to be mentioned in the RFI and
agency is to be informed.

g. The RFI for the same work shall be submitted by the agency again for checking after
rectification as required.

h. The passed RFIs are to be enclosed with RA Bills by the agency, for recommendation of the
consultant for Payment.

i. Cancelled RFIs should be available in the Consultants office.

4.2. Documentation:

The supervision Consultant shall maintain following documents and Registers in their office.

Chapter 5 - Conclusion:
The QAP shall be adhered for carrying out the work and cases such as Non-Conforming Tests,
additional tests, emergency works etc shall be dealt separately. The supervision methodology shall
be revisited in case of multiple shift works and from case to case basis. The Contractor shall maintain
all testing records in his office and each test should be signed by the Consultant/Client.
