I would like to apply for Schengen visa upon my intention to travel to France and Spain from June 1 to
June 8’ 2019.

My purpose of travel is tourism, to witness in actual the glorious sights, particularly the architecture,
churches and museums. I also look forward to experiencing the food, and taking photos of the beautiful
architecture and sceneries in both countries.

I will be accompanied by my husband who has a Canadian Passport. We will be travelling together as part
of his vacation from work.

Please find the following documents to support my visa application enclosed herewith:
1. Application form, duly dated and signed with passport size pictures
2. Passports showing my travel experience and approved visa from USA, Canada, Japan, South
Korea.
3. Employment and leave certificates showing approved leaves of my husband.
4. ITR, credit card statements and bank certificate as proof of income
5. E-ticket for my flight and retrun flight via Qatar Airlines for Manila- Doha-Barcelona and Iberia
for Barcelona- Paris
6. Hotel booking reservation as proof of accommodation
7. Planned itinerary for the trip
8. Travel insurance from Starr Assistance
