MISSION STATEMENT, GOALS, AND OBJECTIVES

The mission of our health promotion program is to provide the elderly of St. Francis commons with the knowledge and primary prevention activities to
reduce the incidence of Alzheimer's.

2. Goals and SMART Objectives

Program Goal #1 To reduce the risk for Alzheimer’s within the elderly community at St. Francis Commons

Process objective(s) During the program, once a month, program planners will test the cognitive ability of each participant and record how many
memorizing games they can complete.

Impact – Learning objective(s)

Learning objective(s) - Awareness level After St. Francis Commons individuals go through the program, they will be able to identify
signs and symptoms of Alzheimer’s

Learning objective(s) - Knowledge level Seniors at St. Francis commons will be able to identify the risk factors contributing to
Alzheimer’s.
Seniors at St. Francis commons can demonstrate cognitive strengthening exercises
properly.
Learning objective(s) - Attitude level During one of the program sessions, 50% of the participants will defend that they know
how to have low risk of Alzheimer’s

Learning objective(s) - Skill development After showing participants what cognitive strengthening programs look like, individuals
level should be able to implement certain criteria to therefore decrease risk factors.
Impact - Behavioral objective Six months after the prevention programs implementation, at least 60% of participants who have completed the
given classes and activities will demonstrate cognitive activity three times per-week, will show higher memorization
skills, and will show lower confusion
Impact - Environmental By the end of six months after program implementation, participating senior citizens will be able to gain access to
objective knowledge and prevention opportunities at a moments notice.

Outcome objective By the year 2020 the county of Oswego will have a 25% lower rate of cognitive impairment.
Program Goal #2 Increase the overall knowledge and education of dementia prevention in the elderly population at St. Francis Commons

Process objective(s) Have the seniors perform a self evaluation of their medical status and lifestyle choices to identify their most pressing risk
factors

Impact – Learning objective(s)

Learning objective(s) - Awareness level After going through this program, St. Francis Commons individuals will be able to tell
others, and understand how to prevent dementia in their age group.

Learning objective(s) - Knowledge level St. Francis Commons individuals will be able to list ways to prevent dementia

Learning objective(s) - Attitude level During one of the program session, 75% of the individuals at St. Francis Commons will
defend that they know enough information about dementia that they don’t need our help.

Learning objective(s) - Skill development After explaining and showing participants what dementia is, how to prevent it, and how to
level identify the risk factors, individuals should be able to tell others their age group how to
prevent dementia; what and what not to do.
Impact - Behavioral objective Six months after the program has ended, 75% of individuals will be on good track for mental health and will be able to
list reasons how to stay there
Impact - Environmental By the end of the year, all seniors involved in the program will be able to perform health evaluations on their time
objective either within their own home, or at the doctor’s office.

Outcome objective By the year 2020, reduce the risk factors of dementia in the elderly of Oswego county to nearly 20% less than what it
is now.
