1. Assemble your equipment :
Basin of water and mild soap or cleaning solution, wash or gauzed
pads, paper or plastic bag for waste, disposable gloves
2. Wash your hands.

3. Ask the visitor to leave the room, if appropriate.

4. Tell the client you are going to give him catheter care.

5. Position the client on his back so the catherter and urinary miatus
are exposed. Put on your gloves.
6. Wash the area gently. Do not pull on the catheter, but hold it with
one hand while wiping it with the other.
7. Observe the meatus for redness, swelling, or discharge.

8. Wipe away from the meatus. Wipe from the meatus to anus

9. Wipe one way and not back and forth.

12. Apply lotion or powder in small quantities to the thighs. Ask your

Collecting a Routine Urine Specimen

RATIONALE: A properly collected specimen allows a laboratory to
evaluate the chemical structure of the urine and evaluate for possible
further testing.
1. Assemble your equipment:
Bedpan or urinal, disposable gloves, measuring container for
measuring output, urine specimen container and lid, paper or plastic
bag for toilet tissue, label, wet washcloth, towel
2. Prepare the label. Write clearly the clients name and address, date,
and the time. Also write what type of specimen this is - routine urine.
3. Wash your hands and put on gloves.

4. Ask visitor to leave the room, if appropriate.

5. Tell the client a urine specimen is needed. Explain the procedure to


him. If he is able to collect the specimen himself, he should do so.
6. If the client is able, he can urinate directly into the container. If he is
not, ask him to urinate into bedpan or urinal. Remind the client to put
toilet tissue not into the bedpan but into a paper bag or a plastic bag.
You will discard the tissue in the toilet. Remove gloves and wash hands.
7. Offer the client a washcloth and towel to wash his hands

8. Make the client comfortable.

9. Put on gloves and take the bedpan or urinal into the bathroom

10. If the client is on I&O. Pour the urine into a clean measuring
container and note the urine amount on the I&O sheet.
11. Pour the urine into the specimen container. Fill it 1/3 full.

12. Put the lid on the container. Wipe off the outside of the container.
Secure the label to the container.
13. Pour the urine remaining in the bedpan, urinal, or measuring
container into the toilet.
14. Clean and rinse the bedpan, urinal, oreasuring container and return
to their proper place.
15. Remove gloves and wash your hands.

16. Make a notation on the client chart that you collected the
specimen, the time, and anything you observed about the client during
this procedure.
17. Store the specimen in the correct place before it is taken to the
laboratory.

Obtaining a Urine Specimen from a Client with a Foley Catheter

RATIONALE: A properly obtained urine sample allows a laboratory to
evaluate the chemical structure of the urine and evaluate for possible
further testing.
1. Assemble your equipment: Specimen container and lid, Measuring
container, label, Disposable gloves, padding to protect the bed,
protective cap for drainage tubing or sterile gauze pads
2. Prepare the label. Write clearly the clients name, address, the date
and time. Write the type of specimen and how it was obtained.
3. Wash your hands.

4. Explain the procedure to the client.

5. Put on gloves.

6. Clamp indwelling urinary catheter below the port by folding plastic


tubing in half and applying a metal/plastic clamp.
7. Wait no more than 15 minutes for small amount of urine to collect in
the tubing above the port.
8. Insert a sterile syringe gently into the port (after swabbing with
alcohol) taking care not to poke the needle through the tubing.
9. Withdraw approximately 10 cc of urine.

10. Insert the urine into the sterile specimen cup. Take care not to
touch the rim or inside cover of the cap ( prevents contamination of the
specimen).
11. Unclamp the tubing and straighten it out. Be sure that the urine is
now freely flowing.
12. Apply the correct lab label the specimen cup and bag according to
policy.
13. Report the result of the procedure to your supervisor.

specimen, the time, and anything observed about the client during this
procedure.
16. Store the specimen in the correct place before it is taken to the

1. Assemble your equipment:
Clean-catch kit or sterilized jar, sterile cleansing solution, sterile gauze,
disposable gloves, bedpan or urinal if the client is unable to go to the
bathroom, wet washcloth, towel, waste bag
2. Prepare the label if it is outside the kit. If not, wait until end of the
procedure. Write clearly the clients name and address, the date, and
the time. Write the type of specimen and how it was obtained.

4. Ask the visitor to leave the room, if appropriate. You may want to
explain this procedure to a family member.
5. Tell the clue t you need a midstream clean catch urine specimen.

6. Explain the procedure. If the client is able, he may collect the
specimen himself.
8. Open the disposable kit.

9. Put on gloves. Remove the towelettes and urine specimen container.


Do not put your hand inside the container or lid.
10. For female clients:
a. Separate the folds of the labia and wipe with one towellette from the
front to the back along one labia. Throw away the towelette. The labia
must be separated during cleansing and collection of the specimen
b. Wipe the opposite labia with the second towelette. Throw it away
c. Wipe down the middle using the third towelette. Throw it away.
11. For male clients:
a. If the male is uncircumcised, pukk the foreskin of the penis back
before cleansing the penis. Hold it back during during urination.
b. Use a circular motion to clean the head of the penis. Use all three
towelettes. Throw each one away after you use it.
12. Ask the client to start urinating into the bed pan or the toilet. Then
ask him to stop. Place the sterile urine container under the stream of
urine and ask the client to start urinating again. Fill the container 1/2 to
3/4 full. The remaining urine may be discarded.
13. If the client is on I&O, all the urine must first voided into a sterile
measuring container and measured, then put into the sterile specimen
container provided. Be sure to note on the I&O sheet the amount of
the urine sent as a specimen.
14. Cover the urine container with the proper lid. Be sure not to touch
the inside of the lid or the container. Wipe off the outside of the
container.
15. Take off your gloves.

17. Make the client comfortable. Offer the client a wet washcloth and
towel to wash his hands.
18. Clean all the equipment and replace it
